Job summary
Jesmond Health Partnership (JHP) is a forward-thinking GP practice operating across four sites in the Jesmond and Gosforth areas. We are seeking a dedicated and compassionate Practice Nurse to support the delivery of high-quality care to our patients as part of our established multidisciplinary team.
The Practice Nurse will play a key role in delivering high-quality, patient-centred care through a variety of nursing services including chronic disease management, immunisations, including travel vaccinations, cervical screening, wound care, and health promotion. The successful candidate will be expected to work autonomously within their professional scope of Practice, as well as collaboratively with the wider clinical team to meet the needs of our diverse patient population.
This is a dynamic role that may require working across all four JHP locations, offering the opportunity to contribute to patient care in a flexible and supportive environment committed to continuous improvement and professional development.

Main duties of the job
The Practice Nurse will support the delivery of high-quality nursing care and contribute to the smooth operation of clinical services. Key responsibilities include:

Job responsibilities
Infection Prevention & Control (IPC)
- Act as or support the IPC Lead, conducting audits, training, and ensuring compliance with policy.
- Support the creation of the IPC Annual Statement with the Practice Manager.
- Assist in reviewing and implementing infection control policies in line with current legislation.
Treatment Room
- Provide support with minor procedures, dressings, ECGs, and patient follow-ups.
- Assist in the completion of medicals and monitor patients as needed.
Clinical Areas
- Monitor cleanliness and compliance across clinical areas.
- Manage infection prevention measures and ensure supply levels are maintained.
- Process samples and oversee vaccine storage, cold chain, and stock control.
- Support the provision of travel health services.
- Ensure adherence to the practice chaperone policy.
Minor Surgery
- Ensure availability of equipment/medication.
- Provide wound care, suture removal, and follow-up management.
- Maintain accurate records and procedure logs.
IUCD Fitting
- Support GPs with IUCD procedures, including patient preparation and follow-up.
- Ensure supplies are maintained and documentation is complete, including patient consent.
Cervical Cytology
- Perform cervical screening in line with national guidance.
- Liaise with local services and attend mandatory training.
- Conduct regular audits and provide advice on contraception, HRT, and womens health.
- Support patient education and monitor related supplies.
Therapeutic Monitoring
- Monitor patient compliance and assess treatment efficacy.
- Recognise clinical abnormalities and recommend adjustments as appropriate.
- Provide care aligned with NICE and NSF guidance.
- Prescribe medications under PGDs within scope of practice.
- Appropriately delegate tasks ensuring safe practice.
Wound Care & Management
- Assess and manage both routine and complex wounds.
- Use diagnostic tools (e.g. Doppler) as required.
- Support the team in managing clinical emergencies.
Health Checks & Chronic Disease Management
- Conduct health assessments and identify lifestyle-related risk factors.
- Develop and evaluate personalised care plans.
- Promote medication adherence and provide education on use and side effects.
- Lead chronic disease clinics and refer to secondary care when necessary.
- Deliver opportunistic and planned health promotion interventions.
- Recognise and refer patients with mental health concerns.
