GP Lead Practice Pharmacist

Job summary

The postholder will lead medicines-related governance across the practice, including incident review, audit activity, action planning and learning events. They will oversee high-risk drug monitoring and ensure compliance with shared care arrangements.

They will maintain robust systems for repeat prescribing, controlled drugs governance and prescribing safety protocols, and will support the practice in meeting all CQC requirements relating to medicines management.

Main duties of the job

The postholder will deliver structured medication reviews for patients with complex polypharmacy, frailty, multimorbidity and high-risk prescribing. They will provide independent prescribing within their scope of competence for both acute and long-term conditions and will contribute to the safe and effective management of repeat prescribing, including acute requests, synchronisation and streamlining of medicines.

The role requires close collaborative working with GPs, nurses, nurse practitioners, advanced clinical practitioners, pharmacy technicians and wider multidisciplinary team colleagues to ensure high-quality, coordinated patient care.

The role includes day-to-day leadership of the pharmacy team, pharmacy technicians and prescribing clerks. The postholder will provide supervision, mentorship and competency assessment for pharmacy colleagues and contribute to wider staff training across the practice on medicines processes, prescribing safety and new clinical pathways.

They will also support the induction and ongoing development of new clinicians, including those employed through ARRS roles.

The postholder will use clinical and digital systems including EMIS, Ardens, AccuRx and prescribing analytics tools to deliver safe, efficient and effective care. They will support the implementation and expansion of electronic repeat dispensing and other digital prescribing innovations.

JOB SUMMARY

The post holder will work as the Lead Practice Pharmacist, providing advanced clinical pharmacy services within primary care. The role combines direct patient care, prescribing leadership, medicines optimisation, governance oversight and team leadership.

CLINICAL DUTIES
