An
exciting opportunity has arisen to join the integrated nurse team at Milton
Abbas Surgery in a beautiful area of Dorset.
This
is a unique role which offers the successful candidate the chance to undertake
general practice nurse duties in a clinic setting and also provide
community care to our patients in their own homes. This model of primary care
nursing is especially well suited to a rural setting and is highly regarded by
our patients and partners in healthcare.
Main duties of the job
Deliver core nursing services including injections under PGD/PSDs immunisations (adult &children), cervical cytology, wound care, travel vaccinations and advice, chronic disease management.
Identify, and manage, treatment plans for patients at risk of developing a long-term condition.
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Support patients to adopt health promotion strategies that encourage them to self-care and live a healthier lifestyle.
Deliver opportunistic health promotion to ensure every contact counts.
Ability to identify signs of deteriorating patient, i.e. sepsis/anaphylaxis and take appropriate action.
Provide information and advice on prescribed or over-the-counter medications and regimens, side effects and interactions within remit of knowledge
Recognise, assess and refer patients presenting with mental health needs in accordance with the National Service Framework (NSF) for mental health.
Liaise with, and where appropriate initiate referrals to specialist services, to ensure adequate care and support to patients and carers.
Be alert to the needs of vulnerable adults and children including risk assessment and taking appropriate action.

Undertake risk assessments in practice and community setting
