We wish to recruit a qualified and
experienced Practice Nurse who is experienced in long term health conditions and may
even be a non-medical prescriber, or you could be a qualified and experienced
nurse, who wishes to progress into Practice Nursing.

professional competence of the post holder (additional training will be
provided before any new duties can be undertaken). The post holder will work in accordance with
the Nursing and Midwifery Council Code of Conduct.

- Registered General Nurse and active on NMC Register
- Demonstrate a commitment to continuous professional development
Knowledge, Skills and Abilities
Essential
- Knowledge of the NMC Code of Conduct
- Ability to communicate effectively in the written and oral form to develop and maintain constructive relationships with a variety of audiences
- Ability to be self-motivated and work co-operatively as part of a team. Managing ones own time with a minimum of supervision to meet agreed outcomes and deadlines
- Ability to manage conflicting priorities and ones own time with a minimum of supervision
- Understanding of how to develop and deliver a person centred approach and an ability to assess, plan and review an individuals needs
- A practical knowledge and understanding of Clinical Risk Management and Infection Control Techniques
Desirable
- Knowledge relating to current Primary Care Policy and national legislation
- A practical knowledge and understanding of Clinical Risk Management and Infection Control Techniques.
Experience

- Experience of the needs of patients with long term conditions
- Experience and awareness of accountability of ones own role and other roles in a nurse led service
- Experience of health promotion strategies
- Knowledge of patient group directions and associated policy
- Experience of using Patient Management Systems, e.g. EMIS and Microsoft packages for a range of different purposes

- Experience of identifying determinants on health in the local area
- Awareness of public health issues in the practice area
