Job summary

As a Advanced clinical practitioner within the practice, you will play a senior role in supporting the wider team. Advanced practitioners form a key cornerstone of clinical stability within the service. You will help provide experience, leadership, and support for colleagues while helping the practice maintain safe and effective patient care.

The role requires an outward focus on meeting the needs of our patients. This means working in a flexible and adaptive way, responding to demand, and helping shape care around the needs of our community.

It also requires an inward focus on the strength of the team. The ACP contributes to a supportive environment by sharing knowledge, mentoring colleagues, and helping create consistent access to senior clinical support for the wider workforce.

Main duties of the job

Key Responsibilities

  • Independently assess, diagnose, treat, and prescribe for patients across a range of conditions

JOB SUMMARY

The post holder will be an experienced Advanced Clinical Practitioner acting within their professional boundary to provide autonomous care for presenting patients. This role is expected to develop and evolve over time as the successful post holder gains experience.

The primary care practitioner will act as an independent clinician, managing a broad range of patient presentations in both the practice and community settings. They will utilise their clinical expertise, including independent prescribing, to assess, diagnose, treat and manage patients effectively.

The post holder will attend home visits, including to residential and nursing homes, working collaboratively with staff, families and other stakeholders to produce and share care plans, including with Out of Hours (OOH) services.

They will also assess and treat acute patients presenting in the surgery, both face-to-face and via telephone consultations. In addition to providing high-quality, evidence-based care, they will contribute to the development of clinical practice through mentoring and supporting colleagues and students.

JOB RESPONSIBILITIES

The primary care practitioner will work within their professional boundaries, delivering high-quality care to patients independently, and referring to other healthcare professionals only where necessary. They will be expected to:

  • Conduct visits to patients in their own homes and care homes.
  • Independently assess, diagnose, treat, and prescribe for patients with a range of acute, non-acute and chronic medical conditions.
  • Utilise independent prescribing competencies to initiate and review treatment plans.
  • Refer patients, as clinically appropriate, to other members of the multidisciplinary team or external providers.
  • Collaborate with clinical and non-clinical colleagues to promote safe, efficient and holistic patient care.
  • Mentor and support less experienced colleagues and students, contributing to their learning and development.
  • Develop innovative models and practices to enhance primary care delivery.
  • Contribute to quality improvement initiatives and the overall development of the practice.

CLINICAL DUTIES

  • Independently assess undifferentiated patients, formulate differential diagnoses, and initiate treatment or further investigations as necessary.
  • Use independent prescribing authority to issue medications and monitor treatment effectiveness.
  • Maintain accurate and timely clinical records using EMIS Web.
  • Undertake and interpret diagnostic tests and clinical procedures (e.g. ECGs, venepuncture, BP monitoring).
  • Deliver individualised care plans and manage chronic disease registers as appropriate.
  • Work within relevant policies and clinical governance frameworks.

COMMUNICATION

  • Communicate effectively and sensitively with patients, carers and colleagues.
  • Deliver complex or potentially distressing information with empathy and professionalism.
  • Build strong working relationships with community, statutory and voluntary organisations.
  • Actively participate in team meetings, staff briefings and clinical discussions.
  • Provide educational sessions or briefings where appropriate.
