Job summary
We are seeking an experienced and motivated Health Care
Assistant (HCA) to join our friendly GP surgery team on a part-time basis. The
postholder will work closely with and support the Learning Disabilities
Advanced Nurse Practitioner (ANP), delivering high-quality, person-centred care
to patients with learning disabilities and complex needs.
The role will involve both practice-based and community
work, including care home visits and home visits. The successful candidate must
be confident working independently, managing their own workload, and providing
compassionate care.

Main duties of the job
- Perform venepuncture (taking bloods) confidently and safely.
- Carry out diabetic foot checks and record findings accurately.
- Provide wound care, including dressing changes and swab collection.
- Collect and process urine samples and other specimens.
- Undertake health checks and monitoring for patients with learning disabilities and long-term conditions.
- Conduct home visits and work within care homes, supporting patients with complex needs under the supervision of the ANP or GP.
- Support and facilitate referrals to a variety of health and social care services.
- Plan and manage own workload, including scheduling home visits and clinics.
- Set up and arrange clinics, ensuring appropriate equipment and documentation are available.
- Monitor and manage practice emails, upload patient information, and respond appropriately.
- Maintain accurate and timely clinical documentation in line with practice and NHS guidelines.
- Contribute to the smooth running of the surgery and support other team members as required.
