Main duties of the job

This role offers an excellent opportunity to make a real difference in medicines management, patient safety, and quality improvement while supporting general practice within a thriving Primary Care Network in North East London. We are looking for a motivated and adaptable Clinical Pharmacist (Band 7) with a passion for delivering high-quality, patient-centred care in primary care settings.

As a Clinical Pharmacist in Primary Care, you will play a vital role in:

  • Optimising medication use to ensure patients receive safe, effective, and evidence-based treatments.

  • Reducing avoidable hospital admissions by identifying, managing, and resolving medicine-related issues.

  • Supporting general practice teams to improve prescribing safety, manage workloads, and enhance patient care.

  • Providing clinical input and leadership on medicines safety, optimisation, and quality improvement projects.

  • Working holistically with patients, taking into account public health and social care needs in medication-related decision-making.

About us

E4 Primary Care Network (PCN) is a thriving collaboration of five GP practices in Chingford, within the London Borough of Waltham Forest. Together, we serve a registered population of more than 53,000 patients, working collectively to deliver high-quality, efficient, and sustainable primary care. Our Network is committed to improving health outcomes, enhancing patient experience, and supporting staff wellbeing through innovative, patient-centred services.

At the heart of our clinical workforce is a dedicated Pharmacist team, comprising three Senior Clinical Pharmacists and two Band 7 Clinical Pharmacists. This team plays a vital role in optimising medicines use, improving safety, and supporting long-term condition management across our practices. Senior Clinical Pharmacists not only lead by example in clinical delivery but also provide mentorship and line management to Band 7 colleagues, ensuring professional growth and consistency of practice.

Our Pharmacists are fully integrated into the wider Network team and work closely with GPs, nurses, and other healthcare professionals. To provide an equitable and balanced service, each Pharmacist is allocated to a maximum of two practices per rota rotation. This model fosters continuity, collaboration, and quality improvement across the Network while allowing Pharmacists to develop strong working relationships within practices.

E4 PCN is proud to be forward-looking, supportive, and committed to developing its workforce.
