Job summary

This is an exciting opportunity to join The E4 Primary Care Network Ltd.

You will be an integral part our team, working alongside our Clinical Director, PCN Manager and PCN Service Delivery Manager to provide efficient and proactive support to the developing services that we work with our practices to provide.

We are seeking a motivated and hard working individual to fulfuil the role of Network Coordinator who will have a key role in supporting delivery of the new Network Contract DES Service specifications, Supplementary Network Services, Impact and Investment Fund and the Enhanced Access Services.

Main duties of the job

The role will be GP and other health and social care professionals facing, with the core responsibility being excellent patient care and outcomes. The coordinator will be a key point of contact within the E4 PCN and will be responsible for ensuring effective communication, information sharing and coordination of care between services, all groups of staff, patients and PCN practices.

The Co-ordinator will be responsible for ensuring that patients across the E4 PCN are identified as appropriate, providing a single-point of access for staff & service users, actively managing patients care delivery. This will require liaison with stakeholders and management of a central search systems.

The Care Coordinator will work with the core PCN team to prepare for meetings clarifying and recording actions agreed at meetings and ensuring the up-dates on actions agreed are communicated to the team and accurately recorded and followed up on clinical systems. This will include the monitoring and handling of sensitive and complex personal information and data compliant with information governance

About us

Our PCN has a population of around 53,000 people in Chingford, London.

The Primary Care Network includes Chingford Medical Centre, Churchill Medical Practice, The Old Church Surgery, The Ridgeway Surgery and Handsworth Medical Practice. The practices have a very strong relationship with one another.

We have an interesting and diverse population, ranging from the young to the elderly. We also support over 300 residents who are living in local Care Homes.

This role would involve working across with all practices and as part of our new and developing Enhanced Access Service.

Proactive management of PCN service deliverables.

  1. Facilitate and ensure the effective delivery of the SNS and DES contracts monitoring progress and reporting outcomes, contributing to the KPIs within appropriate time frames

    2. Explain the management of the Network IIF and SNS to clinical staff, liaising between services and the practices.

    3. Work closely with all relevant care agencies (primary care, secondary care, community services, Mental Health, Social Services, Ambulance Service, Voluntary services and other relevant service providers) to ensure a coordination of services.

    4. Maintain accurate records and statistical returns as required by the CCG, including providing patient-related information for entering into Clinical Reporting Systems, within the required time frame.

    5. Timely process of referrals from the Enhanced Access Services to onward providers of care such as Secondary and Community care providers.

    6. Ensure that appropriate coding is recorded into patient records.

    7. Collect data on patients/carers for recognised outcome measure and document for service interpretation. Ensure all patient notes are updated to reflect any changes, including details on plans

    8. Managing operational meeting processes, identifying areas for discussion and working closely with clinicians and practice managers to define and lead the meetings. Organise and attend relevant meetings when required.

    9. Ensure that meeting actions are recorded, disseminated and followed up in a timely way; ensure relevant practitioners are aware of meeting decisions and actions / outcomes, and chase for action resolution and update.

    10. Network and develop strong relationships with all levels of the NHSs key local players including the CCG, GPs and other primary care contractors, Social Services, Mental Health Trusts, Community Trusts, and other providers including the voluntary sector

    11. Be a contact point for GPs / practices and establish systems and processes which will ensure a timely and appropriate response to queries from clinicians and other stakeholders

    12. Reviewing appointment books to ensure that accuracy is applied and maintained at all times.
