Job summary
Fountain Medical Centre is looking for a Patient Care Navigator who is motivated, hardworking, caring and with a good sense of humour to join our forward thinking practice team.
We are a large resilient and progressive practice based in the town of Newark on Trent, with a list size of 13000 patients and are active members of the Newark & Sherwood Primary Care Network.
Main duties of the job
The following are the core
responsibilities of the care navigator. There may be on occasion, a requirement
to carry out other tasks; this will be dependent upon factors such as workload
and staffing levels:
a.
Process telephone and face to face requests for appointments
following the Practice Active Signposting protocol.
b.
Answer incoming phone calls, transferring calls or dealing with the callers
request appropriately
c.
Signpost patients to the correct service
d.
Initiating contact with and responding to, requests from patients, team members
and external agencies
e.
Read code data on Systm1
f.
Photocopy documentation as required
g.
Data entry of new and temporary registrations and relevant patient information
as required
h.
Input data into the patients healthcare records as necessary
i.
Direct requests for information i.e. SAR, insurance / solicitors letters and
DVLA forms to the administrative team
j.
Manage all queries as necessary in an efficient manner
k.
Carry out system searches as requested
l.
Maintain a clean, tidy, effective working area at all times
m.
Monitor and maintain the reception area and notice boards
n.
Support all clinical staff with general tasks as requested
About us
Overview of your
organisation
Our practice team consists of 4 GP Partners, 3
salaried GP's, 1 Nurse Practitioner and 4 Practice Nurses and Phlebotomist.
We
have 7 patient care navigators, who are managed by our reception manager, a
medical secretary and four data entry administrators who provide support to the
practice team.
We are a supportive team who provide excellent care
to our 13000 patients.
