Job summary

Trent Meadows is seeking an experienced Primary Care Pharmacist to become a valued member of our dynamic Pharmacy team.

About You:The ideal candidate will have completed the CPPE (or will need to commit to completing) course, hold prescriber status, and possess a minimum of three years post-qualification experience. You will be confident managing long-term conditions, working directly with patients to assess and treat ailments, and encouraging self-care. A solid understanding of Structured Medication Reviews (SMRs) and the strategic delivery of primary care targets is essential.

Key Skills:You will demonstrate excellent verbal and written communication, have experience influencing clinical decisions, and be skilled in conducting medication reviews.

What Were Looking For:Experience with EMIS and Docman is preferred. Strong interpersonal and communication skills, flexibility, teamwork, and excellent organisational abilities are essential. Above all, you should be passionate about primary care.

Main duties of the job

As part of a multidisciplinary team, you will help develop and manage key processes such as repeat prescription reauthorisation, medicines management during care transitions, and safer prescribing systems. You will collaborate closely with GPs, Advanced Nurse Practitioners (ANPs), and nurse prescribers who will provide support.

Job responsibilities

Clinical Pharmacist who acts within their professional boundaries, working alongside a team of pharmacists in general practice within the wider PCN.

They will improve patients health outcomes and the efficiency of the primary care team by providing direct, accessible and timely medicines and expertise including face to face contact with patients.

The post holder will be an integral part of the general practice team, as well as part of a wider pharmacist network and multi-disciplinary team. This includes being a conduit of patient medicines information into and out of UHDB. They will work to optimise medication issues to improve patient care and safety.

As part of their employment, the post holder will achieve qualifications from an accredited training pathway, including independent prescribing, equipping the Clinical Pharmacist to be able to practise and prescribe safely and effectively in a primary care setting.As part of a Lead Pharmacist role the post holder will undertake Medicines reconciliation in line with practice protocols and standard operating procedures to improve the efficiency and effectiveness of prescriptions and repeat prescriptions.

The post holder will help patients to get the best from their medicines by communicating changes agreed and approved protocols, improving repeat prescribing processes, promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines. Essential

  • The Clinical Pharmacist is enrolled in, or has qualified from, an approved 18-month training pathway or equivalent that equips them to:
  • Be able to practice and prescribe safely and effectively in a primary care setting (for example, the CPPE clinical pharmacist training pathways)
  • Although optional for organisations other than a PCN, it is thought advisable that they are able to deliver the key responsibilities outlined in section B1 of the Network Contract Directed Enhanced Service 2024-25 specification.
  • GPhC registered pharmacist
  • Hold or be working towards an GPhC independent prescribing qualification
  • Minor ailments certification
  • Membership of the Royal Pharmaceutical Society
  • Working towards faculty membership of the Royal Pharmaceutical Society
  • Medicines management qualification
  • Flexibility to work outside core office hours
  • Disclosure Barring Service (DBS) check
  • Evidence of continuing professional development

Desirable

  • Access to own transport and ability to travel across the locality on a regular basis, including to visit people in their own home
