Care Navigator (Receptionist) GP Practice

Job summary

We are looking for a full-time or possible job share Care Navigator to work 5 days per week, within a very busy two-partner GP surgery.

We are a friendly, all inclusive team that values all members of staff and passionately believe in providing excellent patient care

Main duties of the job

Duties: Engage with patients both via the telephone and in person. Book appointments and process prescriptions. Maintain accurate patient records using clinical systems like EMIS Web and Docman which includes scanning and raising referrals. Liaise with other healthcare professionals to ensure seamless care coordination. Direct patient enquiries to the appropriate healthcare professionals. Register new and temporary patients.

About us

Apex has a relatively flat structure with two GP partners, Clinical Pharmacists, Practice Manager, Assistant Practice Manager, a Nursing team and an admin team. Team work is critical to the culture of our surgery.

Job responsibilities

As a Receptionist/Care Navigator you will be the first point of contact for patients, ensuring they receive the right care, in the right place at the right time. Working within a busy GP Practice you will support the clinical teams by managing patients enquiries, signposting to appropriate services, and performing key reception and administrative duties.

Greet patients in person, by phone and online

Essential

  • Excellent communication skills
  • Compassionate and patient-centred
  • Ability to work under pressure
  • Good IT skills and accuracy

Desirable

  • Previous experience of working in a GP practice
  • Familiarity with clinical system such as EMIS Web and Docman

Qualifications

  • GCSE A to C in English and Maths.
  • Competent computer skills
  • Team player with a proactive attitude

Desirable

  • Experience within a GP surgery preferable but not essential.
  • Experience with EMIS Web
