Advanced Nurse Practitioner

Job summary

Responsible for attending to patients who wish to be reviewed by a health care professional; diagnosing and treating patients presenting with a variety of illnesses, seeing and advising people in respect of their continuing medical and nursing needs. These patients will be a combination of telephone triage calls and booked appointments. The nurses will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner. The nurses will be responsible for advanced care planning.

Main duties of the job

• Assess, plan, provide and evaluate nursing care to meet the care needs of individuals in the practice population. • Assess, examine, diagnose and treat specific conditions in accordance with agreed medical/ nursing protocols, and within own area of competence or speciality. • Provide access to GP’s as necessary. • Undertake diagnostic, health screening and health promotion at consultation/ opportunistically. • Triage and treat patients wishing to see a health care professional making any necessary referrals to other members of the primary health care team • Provide specific evidence based practical skills and knowledge in telephone and clinics. • Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations. • Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary • Undertake the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures requested by the GPs • Report on and action results

About us

We are a small to medium sized practice based in a purpose built medical centre in Grimsby and also have a branch surgery in the village of Laceby. The practice team consists of a variety of staff to assist the GP's which includes Advanced Nurse Practitioners, Practice Nurse, Health Care Assistant and the support of the administrative team. We also have access to wider community services such as the Community Nurse, Complex Care Team, First Care Practitioners, Dietitians. We are looking for an enthusiastic Nurse Practitioner to join our practice team.
