Job summary

We are looking for an experienced and motivated Practice Nurseto join our friendly primary care team on a 12-month maternity cover contract. The role is suitable for a Band 6 or Band 7 nurse, depending on experience and scope of practice. You will provide high-quality, patient-centred nursing care and contribute to the effective management of long-term conditions within the practice.

Main duties of the job

Clinical Responsibilities

  • Provide high-quality nursing care to patients across the practice population

  • Undertake chronic disease management clinics (e.g. asthma, COPD, diabetes, hypertension)

  • Deliver cervical cytology in line with national screening guidelines

  • Administer childhood and adult immunisations and vaccinations

  • Carry out health promotion and disease prevention activities

  • Manage long-term conditions in accordance with evidence-based protocols and QOF requirements

  • Support infection prevention and control standards within the practice

  • Maintain accurate and timely clinical records using the practice clinical system

  • Contribute to clinical audits, service development and quality improvement initiatives

  • Participate in policy development and review
