Job summary

Kingswinford Medical Practice is seeking to recruit a caring, understanding and hard working Practice Nurse to join our Clinical team.

We are open to taking applications from newly qualified nurses to experienced nurses. Staff are encouraged to engage in training relevant to the post after discussion with the GP Lead. We have an experienced and supportive team who will also help develop skills.

We have an experienced clinical team consisting of 4 Practice Nurses, an ANP, HCA, Paramedic, 3 GP Registrars and 5 GP partners in addition to a number of ARRS roles.

The clinical team are supported by experienced Administrative, Reception and Management teams.

Systems used include EMIS Web, Docman, ICE, AccuRx and Silicon Footfall.

The successful candidate will possess excellent communication, computer, and interpersonal skills, with the ability to work efficiently under pressure and as part of a collaborative team.

The position is on a part time basis (15-25 hours per week), the successful candidate will be expected to work until 7:30 / 8pm on either Wednesdays or Fridays.

Main duties of the job

This role is pivotal in delivering comprehensive healthcare services.

The following are the core responsibilities of the Practice Nurse:

a. To develop, implement and embed health promotion and wellbeing programmes

b. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

c. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health

d. To provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

e. As required, to diagnose and manage acute and chronic conditions

f. Provide wound care to patients

g. To review patient medications to enhance compliance andprovide guidance and support to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages(within own scope of practice)

h.To co-ordinate the provision of travel medicine services

i. To request pathology services and process pathology results as required

j. To maintain accurate clinical records.

k. To undertake removal of sutures, ECGs, venepuncture, hypertension management, ear syringing and cytology (training available if required)

l. To implement vaccination programmes for adults and children.

m. Chaperoning and assisting patients where appropriate

About us

Kingswinford Medical Practice is a partnership of five General Practitioners. It is a well established surgery and, because of its geographical location, serves the communities of Kingswinford, Wall Heath, Pensnett, Brierley Hill, Wordsley and the nearby villages of Swindon and Hinksford.

Our staff are an enthusiastic team of professionals who aim to provide the best service possible for all our patients. The practice prides itself in encouraging a positive attitude towards good health and will always try to be flexible and accommodating to patients' needs.

We are a friendly and supportive team of 30 who serve 8600 patients. We are an active participant in the Kingswinford and Wordsley PCN and also host a range of additional healthcare professionals including a paramedic practitioner, pharmacists, first contact physiotherapists, an Occupational Therapist and a Health and Wellbeing Coach.

aa. To support the clinical team with all safeguarding matters, in accordance with local and national policies

bb. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

cc. To deliver opportunistic health promotion where appropriate

In addition to the primary key responsibilities, the Practice Nurse may be requested to:

a. Develop and review audit protocols and process for the practice, effectively utilising the audit cycle

b. Develop, review and implement nursing protocols in conjunction with the partners

c. Support all members of the nursing team, providing guidance when necessary.

d. Participate in local initiatives to enhance service delivery and patient care

e. Support and participate in shared learning within the practice in order to improve patient care

f. Continually review clinical practices, responding to national policies and initiatives where appropriate

g. Participate in the review of significant and near-miss events applying a structured approach, i.e., root cause analysis (RCA)
