Job summary

The Rural West Primary Care Network (PCN) is seeking to recruit an experienced Clinical Independent Prescriber (IP) Pharmacist who thrives on team working to join our friendly, busy network of Practices. The post holder will work within their clinical competencies as part of a multi-disciplinary and multi-site team to provide expertise in clinical medicines management. This includes provision of structured medication reviews of patients with polypharmacy, supporting the management of long term conditions through medicines optimisation and the management of medicines on transfer of care, as well as systems for safer prescribing. The post holder will address the public health and social care needs of patients in the PCN to improve patient care by interfacing with community and hospital pharmacy colleagues, helping people to stay well and at home.

Main duties of the job

The post holder will use their expertise to provide clinical medicines reviews and undertake duties in medicines management, including reduction of inappropriate polypharmacy, contributing to reductions in hospital admissions, and providing medication information to patients and healthcare professionals. They will also manage repeat prescription authorisations, reauthorisation and acute prescription requests. The post holder will also take part in quality improvement initiatives and clinical audit as well as supporting the Practices' Quality and Outcomes Framework activities.

We are looking for a Clinical Pharmacist who already holds an Independent Prescribing qualification and ideally is already accredited from the required 18 month CPPE Primary Care Pharmacy Education Pathway. Pharmacists who have already completed a previous CPPE pathway, or have a considerable amount of prior knowledge, skill and experience that they can demonstrate, may be exempt from part/all of the Pathway. For those requiring this training, the PCN will provide full support.

Applicants will need excellent communication, consultation and interpersonal skills and have experience working as an autonomous Clinical IP Pharmacist, preferably in a similar PCN role. They will also be able to demonstrate strong team working skills and support for their colleagues.

The Rural West Primary Care Network (PCN) comprises two GP Partnerships: Tadley Medical Partnership and Watership Down Health with surgeries in Kingsclere, Overton, Oakley and Tadley in North Hampshire. The role is based within both Partnerships' Practices in the PCN.

Job responsibilities

The role spans the following areas;

Reduce inappropriate polypharmacy and wasteful prescribing through clinical medication review

Reconcile medicines following hospital discharge and work with patients and community Pharmacists to ensure patients receive the medicines they need post discharge

Contribute to reductions in medicine related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines related issues

Interface with community and hospital Pharmacy colleagues and develop referral processes between primary care professionals including the promotion of the repeat dispensing service

Manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates

Support the Practices and PCN in achieving QOF and IIF targets and other quality initiatives related to prescribing

Manage patients medicine queries and educate patients on their medicines, promoting self-management and adherence

Provide specialist advice and support to GPs, nurses, and other healthcare professionals on medication-related issues.

Increase quality and safety of prescribing through mechanisms such as audit and PDSA cycles

Manage Practice formularies to improve the quality, safety and cost effectiveness of prescribing

Implement drug withdrawals and alerts e.g. MHRA aimed at improving medicines safety

Work with primary care professional and patients to implement NICE and other evidence based guidelines and prescribing policies, in line with local and national recommendations

Run clinics where medicines are the main interventions e.g. anticoagulant monitoring clinics.

Contribute to multi-morbidity reviews and multidisciplinary reviews

Provide medicines information and training to Practice healthcare professionals and admin staff

Review daily Pathology results for patients on known medicines

Act as a source of medicines information for all the Practices teams and patients (e.g. around doses, side effects, adverse events, possible alternatives e.g. around out of stocks)

Undertake minor ailments triage: dealing with minor ailments and triaging patients appropriately

Undertake Structured Medication Reviews (SMRs) for the required patient cohorts

Support the wider Practice/PCN multidisciplinary teams to deliver NHS vaccination services (e.g. Flu, Covid-19 vaccinations etc.) to the PCN population
