Job summary

The Rural West Primary Care Network (PCN) is seeking to recruit an enthusiastic, highly motivated and experienced Digital and Transformation Lead to join our friendly busy network of practices. The Network comprises two GP Practices, Tadley Medical Partnership and Watership Down Health with surgeries in Kingsclere, Overton, Oakley and Tadley, Hampshire.

The post holder will work with the Clinical Directors and the Leadership within Rural West PCN to deliver on our vision to work with our communities, share best practice and improve health outcomes. They will plan, co-ordinate and directly support delivery of transformation activity within the PCN, which is based on data to support quality improvement, prevention and population health. The use of data and digital tools to proactively influence the way in which people engage with healthcare and their health is core to this role and the evolution of primary care.

Main duties of the job

The post holder will work closely with both the Practices and PCN staff to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered. They will develop relationships with peers across other PCNs to share learning and expertise at place or system level. They will also develop relationships with the ICS to discuss and agree local transformation plans and activity and to support alignment with broader strategy, including the ICSs digital strategy. Working with relevant colleagues in the ICS, the post holder will develop relationships with local delivery partners (such as IT service providers and other NHS organisations) to support effective delivery of transformation.

Working with PCN leadership, the post holder will help to develop strategy related to transformation and improvement, aligning this with larger scale strategy and initiatives. They will contribute to collaborative problem solving with other system partners, patients and communities; focusing their time on the planning, implementation and evaluation of practical improvements that make a difference to patients and staff in the PCN.

The post holder will lead and develop a strategic plan to build a robust digital infrastructure for Rural West PCN and to implement this, which;

Leadership and Management

1. To develop and maintain an in-depth understanding of the PCN digital infrastructure and business intelligence tools in conjunction with existing practice staff

2. To ensure all change is ultimately focused on improving services for all patient groups

3. To research digital tools for the PCN which is aligned to a framework of fundamental principles and seek to procure, in conjunction with Practices, in the most cost effective way, respecting that both Partnerships wish to remain separate

4. To proactively project manage all digital projects and also evaluate the effectiveness of the intervention

5. To ensure all initiatives are supported by a clinical champion and Practice management

6. To lead the development and implementation of digital transformation bids

7. To ensure all staff receive training on the digital tools available within the PCN

8. To ensure the induction of all new members of staff receive training and resources on tools available

9. To ensure all initiatives are fully scoped and underpinned by a quality improvement methodology

10. To remain up to date with the national and local PCN guidance

11. To be responsible for supporting the PCN to identify and make progress with digital transformational change, understanding and interpreting national and system-wide policy developments and initiatives to local circumstances and contexts
