medical Partnership with a list size of circa 24,000 patients on the edge of
the beautiful New Forest. We strive to provide excellent care to our patient
population, whilst maintaining a focus on staff wellbeing and supporting our
team.
Working at New Horizons Medical
Partnership you will be part of our team made up GPs, ANPs, practice nurses
and HCAs. We also have a full mental health team, pharmacists, care
co-ordinators and a physio. Geographically our patients are well served by both
the hospitals in Southampton and Lymington.

Main duties of the job
Job Summary
As an Advanced Nurse Practitioner (ANP)
at New Horizons Medical Partnership you will provide an efficient on the day
service to our patients, working across our current two GP Surgery sites,
Forest Gate Surgery and Totton Health Centre.
Working independently, face to face or
via telephone consultation, you will be accountable for delivering safe,
clinical decision making and expert care to a high standard.
If you have an interest in education,
you could take part in a new to the practice initiative by being involved in the
supervision of student nurses. New Horizons is a GP Registrar training
practice, and we are interested in expanding this to include student nurses.
Salary will be dependent on experience
and will be discussed at interview. Other benefits included for this role are

This is a part-time post working a

About us
Within the ANP team, there is a
clinical lead ANP and a partner lead. We provide supervision sessions, teaching
at a PCN level and monthly clinical team meetings. There will always be a
senior doctor for support, and we practice within safe capacity limits. We
recognise the importance of time to decompress and there is a timetabled team
coffee break part way through every session. Outside of work there are events
organised too.

including initial history taking; clinical assessment; diagnosis; treatment;
prescribing and evaluation of their care.
You will also contribute to the
leadership and mentorship of the treatment room team.
Formulate a management plan for the
patient, using other sources of healthcare professionals (such as GPs or
Pharmacists), if required, to support diagnosis or next course of action.
Prescribe medication as required
within your clinical competence.
Your responsibilities could also
include but not limited to other aspects of Nursing care like Chronic disease
clinics and learning disability clinics. We would welcome any additional
roles/duties that you might like to pursue outside of the on the day service.
You will be professionally and legally
responsible and accountable for all aspects of clinical decision making within
your clinical competence.
You will need
to cover PAN PCN working as and when required.
Training:
Post-holders will be encouraged and
supported to undertake any training required to ensure appropriate role
development.
Personal/Professional
Development:
The
post-holder will participate in any training programme implemented by the
Practice as part of this employment, such training to include:
Participation in an annual individual performance
review, including taking responsibility for maintaining a record of own
personal and/or professional development.
Taking
responsibility for own development, learning and performance and demonstrating
skills and activities to others who are undertaking similar work.
