Job summary
We are a two-site partnership with a practice population of circa
24,000 patients. We are seeking an enthusiastic
Practice Nurse with Chronic Disease Management to join our forward thinking and friendly nursing team.
The focus of the role is to deliver safe and effective nursing care to our patient population. The ideal candidate will be able to work independently and also be able to work as part of the whole general practice team. The person will need to demonstrate strong professional decision making skills and clinical expertise.
This post is subject to the Rehabilitation of Offenders Act

Main duties of the job
The purpose of this role is to;
Deliver practice nursing services and care to the practice population, support patients to live healthily, monitoring of chronic conditions, health promotion and screening activities.
You will undertake a variety of administrative duties to assist the smooth running of the practice including the provision of clerical support to clinical staff and other members of the practice team.
You will also facilitate effective communication between patients, members of the primary healthcare team, secondary care and other associated healthcare agencies.
You will be working at Registered Nurse Level Practice as described in the Primary Care and general practice Nursing career and core capabilities framework.
