Job summary

We have an exciting opportunity for an experienced Advanced Clinical Practitioner (ACP) to join our established Primary Care Network (PCN). We serve the people of Totton and surrounding areas, including Ashurst and Marchwood, with a population of approximately 37,000 patients.

This post as a full time vacancy, working 37.5 hours per week between the core hours of 08:00am - 18:00pm, Monday to Friday. Some evening and weekend hours may be agreed on a rota basis to cover Enhanced Access.

Main duties of the job

The post holder will be an experienced ACP within primary care who is able to deliver quality individualised care to our patients with a high level of autonomy underpinned by the four pillars of Clinical practice, leadership and management, education, and research.

You will be expected to utilise your skills in advanced clinical assessment and diagnostics reasoning to make complex decisions to determine and initiate changes to treatment. A keen interest in acute Frailty or Urgent Care would be advantageous.

The ACP will form a core part of our home visiting service and urgent care team. Additionally, there is scope to provide chronic disease management and routine work dependant on need and candidate interest. Support, supervision, and additional training will be provided where necessary to achieve this.

The ACP will have an MSc in advanced Clinical Practice along with:

  1. Full NMC / HCPC Registration with no restrictions
  2. MSc in Advanced Clinical Practice
  3. Non-Medical Prescriber V300
  4. Experience of working in urgent care and/or primary care.

Currently we are a team of Paramedics, Physiotherapists, Pharmacists, Social Prescribers, Care Co-ordinators and Mental Health workers who work closely alongside our colleagues at Testvale Surgery and New Horizons Medical Partnership.

Job description

Job responsibilities

  1. Work alongside the multi-disciplinary team to provide urgent care and a home visiting service, providing clinical triage when required, in accordance with clinically based evidence, NICE guidance, the Direct Enhanced Services contract of the Network.
  2. As required, be able to prescribe for the caseload of patients within your proven scope of practice. Using your knowledge, you will provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and interactions.
  3. Be able to perform and interpret investigatory procedures, including laboratory and radiology.
  4. Support the delivery of anticipatory care plans and lead certain community services.
  5. Working in alignment with the four pillars of Advanced Practice, provide clinical leadership and supervision to PCN and surgery colleagues.
  6. Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.
  7. Collaborate with other members of the PCN including doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care.
  8. Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.
  9. Review medications following policy, NICE (national), local clinical guidelines and local care pathways. This will be done in accordance with evidence-based practice, national and practice protocols, and within the scope of practice.
  10. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
  11. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  12. Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
Secondary Responsibilities
  1. Enhance their own performance through continuous professional development, keep up to date with current evidence-based practice, impart own knowledge and skills to PCN colleagues to meet the needs of the service.
  2. Provide leadership for members of the multi-disciplinary team, providing guidance and support when necessary.
  3. Participate in research and utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
  4. Work in partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.
  5. Continually review clinical practices, responding to National policies and initiatives where appropriate.
  6. Participate in the review of significant and near-miss events, applying a structured approach, i.e. root cause analysis (RCA).
  7. Take personal responsibility for own learning and development, including the requirement to maintain currency, achieving all targets set in own Personal Development Plan (PDP).
