Job summary

Brentford Group Practice is a well-established GP practice/list size 8200, CQC rated as good with high QOF achievement and we use System One. We are looking for an enthusiastic and dedicated Practice Nurse for 15 hours a week over 2 days. We are looking for someone who shares our commitment and passion for providing high-quality patient care and who also has the desire to attain their own individual career goals.

Main duties of the job

The post holder will provide care for the presenting patient, treatment, chronic disease management and evaluation of care. As well as providing the full range of primary care services to our patients, including immunisation for adults and children, cervical cytology, wound management, Asthma and COPD reviewS, Diabetes review, Swabs, Mental Health Review, Travel Vaccination, Diabetic Checks Health checks, INR, Infection control management/audits etc.

An individual who can work collaboratively with the general practice team to meet the needs of the patients, ensuring that practice policies and procedures are followed.

To be considered for this exciting role you must be a NMC UK registered nurse with significant post-registration experience as well as the Right To Work in the UK

Our list size is 8200 with an interesting mixed demographic. We are active within PCN and our PCN staff consists of pharmacists, first contact physio, a social prescriber, a mental health advisor, a paramedic and a dietician. We are rated Good by CQC and we use system one.

Job description

Job responsibilities

The post holder will provide care for the presenting patient, treatment, chronic disease management and evaluation of care. As well as providing the full range of primary care to our patients including immunisation for adults and children, cervical cytology, wound management, Asthma review, Diabetes review, Swabs, Mental Health Review, Health checks, INR, Infection control management/audits etc.

An individual who can work collaboratively with the general practice team to meet the needs of the patients, ensuring that practice policies and procedures are followed.

As required, to provide routine nursing care to patients in accordance with clinical-based evidence, NICE and the NSF

. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients' health

To implement vaccination programmes for adults and children including for housebound patients

To provide treatment of minor injuries, dressing, removal of sutures

To undertake Cardiovascular Screening in patients aged 40-74 opportunistically. Provide lifestyle advice and education, and refer patients where appropriate to healthy lifestyle programmes.

Maintain health promotion templates updating info on CHD, alcohol, smoking, and obesity, and ensure appropriate registers are maintained for call and recall of patients.

To counsel men and women seeking sexual health advice and initiate sexual health screening in line with the local public health contract

To provide contraception advice service, starting people on COCP, POP and Depo in consultation with the doctors and in accordance with PGD.

Taking of cervical smears. Maintaining a smear recall system and notifying patients of results. To set up regular relevant audits and ensure they are carried out at the appropriate intervals.

Provide an Oral Anticoagulation therapy service for patients, using INR star.

Responsible for maintaining adequate stock of dressings, drugs, specimen bottles, swabs, instruments, diagnostic agents, utensils and leaflets. Responsible for ensuring all consultation rooms are stocked and clinical waste is removed on a regular basis.

To have overall responsibility for maintaining the cold chain, including managing the fridges and ensuring staff understand their roles & responsibilities.

Responsible for maintaining clinical equipment, ensuring there are cleaning & maintenance schedules in place in line with the manufacturers recommendations.

Responsible for maintaining the treatment rooms in a clean and tidy manner and correct disposal of clinical waste.

To maintain accurate clinical records in conjunction with extant legislation

To ensure read codes are used effectively by all members of the nursing team

To ensure the appropriate maintenance and use of chronic disease registers

To provide chaperoning and assisting patients where appropriate who are being examined by another clinician. Ensure compliance with the practice chaperone policy.

To support the team in dealing with clinical emergencies

To recognise, assess and refer patients presenting with mental health needs

To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

To attend regular clinical and practice meetings

To support the clinical team with all safeguarding matters, in accordance with local and national policies and attend safeguarding meetings

To deliver opportunistic health promotion where appropriate
