Care Coordinator (Safeguarding)

Job summary

The Care Coordinator - Safeguarding post provides a rewarding opportunity to work alongside GPs and Practice teams to develop efficient processes and systems at the very heart of general practice.

Provide central co-ordination for safeguarding information and communication to help GPs plan care for vulnerable patients.

Maintain consistent protocols for safeguarding administration, document handling, record management and communication with partner organisations.

Support multi-disciplinary team working - communicating between teams who have a joint role in looking after vulnerable patients; the Primary Care Network team, Multi Agency Safeguarding Hub and the CCG.

Main duties of the job

  • Safeguarding policies -Help the practice to ensure safeguarding policies and procedures are up to date and consistent with agreed best practice
  • Safeguarding Training -Help the practice run training events which encourage a multidisciplinary approach to safeguarding.
  • Information Governance -Provide advice and training on the recording and coding of safeguarding relevant information within the electronic patient record.
  • Safeguarding Assurance -Support the practice in achieving CQC compliance with respect to the safeguarding of staff and patients.
  • MDT facilitator -Enable and support the development and running of an effective practice safeguarding MDT for children and vulnerable adults.
  • Working together -Help the practice foster effective relationships with local safeguarding partners. Ensure the timely sharing of relevant information, both verbally and in a structured report, when safeguarding concerns arise in respect of both children and adults.
  • System Learning -Support the practice in responding to statutory requests for information required by the local authority to carry out local child or adult safeguarding case reviews and assist the practice with section 42 enquiries. Disseminate learning and support the implementation of review recommendations.
  • Referral Pathways -Promote the understanding of local safeguarding referral pathways, including Early Help and support services for victims of domestic abuse.

Job description

Job responsibilities

To oversee the workflow of safeguarding reports and documents and ensure they are circulated to appropriate clinicians in line with practice safeguarding policies.

To triage incoming information from Adult Services in regards to vulnerable adults, including completing MARAC reports, hospital DNA/WBN letters, police SCARFs and any other information relevant to the safeguarding department.

To summarise and code the documents into the relevant specific templates, highlighting to patients registered GPs anything significant.

To ensure the correct coding is added to patient records to aid clinicians in identifying those children and adults who are cause for concern.

To add alerts to parent records, those who have a child on a safeguarding plan.

Any other tasks delegated by practices, suitable to the candidates strengths.

To contribute to data collection and audit in relation to safeguarding to support and improve standards for safeguarding practice across the PCN.

Plan and provide administrative support for the weekly safeguarding huddle, MDT to be chaired by the Senior GP Partner, including minute taking and agenda items.

General

Responsible for reviewing and processing all incoming proxy access requests, which allow a nominated individual (such as a parent, carer, or legal representative) to access a patients medical records via online services. This includes verifying the legitimacy of the request, ensuring appropriate consent and documentation are in place, and assessing any safeguarding risks in line with practice protocols and national guidelines.

Proxy access requests for care homes.

Working with the Duty Manager to ensure patient consent is correctly recorded on SystmOne Online.

Actively participates and takes minutes in Gold Standard Framework (GSF) meetings to support the identification, care planning, and coordination of patients nearing the end of life, ensuring a multidisciplinary approach to high-quality palliative care.
