Job summary
An exciting opportunity has arisen for a practice nurse to
join our friendly practice. The role includes the usual practice nurse duties
such as smear taking, wound care, vaccinations, pill checks and long-term
condition management.
The post holder will be responsible for the implementation
of processes for the effective management of patients with long-term conditions
using evidence-based practice including care for elderly and housebound
patients.
Working as a senior member of the practice multidisciplinary
team, the post holder will ensure nursing services are delivered effectively to
the entitled patient population.
The Practice Nurse will be responsible for a number of
clinical areas such as infection prevention and control, chronic disease
management, health promotion, well women and well man clinics, as well as
actively supporting the practice management team in the reviewing and delivery
of clinical policy and procedure.
Main duties of the job
Main duties:
On occasion, there may be a requirement to carry out tasks
not listed below, that are within the reasonable scope of the role.
Duties will include but are not limited to:
- Undertake cytology
- Care of acute & chronic wounds
- Removal of Sutures/Staples
- Urinalysis for both acute & chronic conditions
- Contraception reviews
- ECGs both acute & routine
- Phlebotomy
- Chronic Disease Management (Diabetes, Asthma and COPD)
- Hypertension Management 24hr Blood Pressure Monitoring and
BP checks in clinic
- Adult & Childhood immunisations
- Prepare & send samples for lab testing
- Chaperoning
- Assisting clinicians with minor procedures
- Support the Infection Prevention Control (IPC) lead
- Assess the needs of patients ensuring the provision of
healthcare is appropriate, incorporating evidence-based practice
- Implement & evaluate individual treatment plans for
chronic disease patients that promote health and wellbeing.
- Identify, manage & support patients at risk of
developing long-term conditions, preventing adverse effects to patients
health.
- Provide routine nursing care to patients in accordance with
clinical based evidence.
- To review patient medications to enhance compliance.
- Co-ordinate
the provision of travel medicine services.
- Ensure read codes are used effectively.
- Prioritise health issues and intervene appropriately.
- To support with any clinical emergencies.
- Recognise, assess and refer patients presenting with mental
health needs.

Peninsula.

- Obtaining appropriate information using a lifestyle questioning approach and identifying appropriate health promotion issues
- Working with patients to develop a management plan where health problems or potential health problems are identified
Risk assessment:
- Recognising issues and gathering sufficient information to refer (e.g. drugs; domesticviolence; child protection; vulnerable adults; senior patients; social problems)
Health screening:
- Recognising issues and gathering sufficient information to refer (e.g. womens and mens health, sexual health, older people)
- Undertaking monitoring tasks (e.g. smears) and providing advice as appropriate
- Providing in depth monitoring and advice as appropriate to the level of the post-holders specialty and working with the patient in deciding on management plans.
