Working Arrangements

This role is up to4sessions per week, with sessions agreed in advance. Some flexibility will be required to attend system meetings or project groups when needed. We can consider candidates wanting to be eitheremployed on a self employed of PAYE basis.

Main duties of the job

The Role

The Clinical Neighbourhood Lead will provide clinical leadership for neighbourhood initiatives, supporting delivery of the NHS Neighbourhood Plan within St Helens South PCN.

Working closely with the PCN Clinical Director, Operational Clinical Lead and Operations Manager, the post holder will oversee neighbourhood projects, support service development and work collaboratively with partners across the local health and care system.

This is an exciting opportunity for a clinician with an interest in integrated care, service improvement and developing new models of care across primary and community services.

  • Provide clinical leadership and oversight for neighbourhood projects.
  • Ensure initiatives are clinically safe, evidence-based and patient-centred.
  • Support the design, implementation and evaluation of neighbourhood models of care.

  • Provide clinical oversight for neighbourhood programmes and projects.
