Job summary
The Care Coordinator will play an important role within the Primary Care Network (PCN), working as part of the multidisciplinary team to deliver high-quality, patient-centred care.
The primary focus of this role is to provide clinical support as a Health Care Assistant (HCA) undertaking tasks such as blood pressure monitoring, blood tests, ECGs, and other routine procedures.
In addition, as part of their role, the post holder will provide secondary support to the Care Coordination team, assisting with administrative tasks, reception cover, and rota coordination to ensure smooth day-to-day operations across the PCN.
This hybrid role requires excellent clinical competence, attention to detail, and strong communication and customer care skills.
Main duties of the job
Clinical (HCA) Duties
- Perform routine clinical observations including blood pressure, pulse, height, weight, and BMI.
- Undertake phlebotomy (blood taking) and record results accurately.
- Carry out ECGs and ensure documentation is completed appropriately.
- Support clinicians with patient preparation, specimen collection, and clinical documentation
- Ensure clinical areas are maintained to infection control and safety standards.
- Record and update patient information accurately on clinical systems in accordance with policy
- Support health promotion and basic patient education under supervision.
Administrative & Care Coordination Support
- Process and effectively signpost patients to the appropriate healthcare professional, depending on the presenting condition
- Answer incoming phone calls, transferring calls or dealing with callers requests appropriately.
- Process patient requests for appointments and repeat prescriptions in line with practice protocols.
- Initiate contact with and respond to requests from patients, team members, and external agencies.
- Enter Read/SNOMED CT coded data
- Assist with rota coordination within the reception and Care Coordination teams.
- Reception duties / cover
- Provide support to practices with MDT meetings and general coordination activities.
- Carry out data entry, system searches, and reporting tasks as requested.
- Support clinical and administrative staff with general tasks as required

The St Helens South Primary Care Network Limited Company (PCN) is the organisation that delivers the core components of the PCN Contract as well as supports the wider Primary Care system and tenders for, designs and develops new/ existing services for the benefit of the St Helens South population.
The PCN is comprised of 10 Practices in the St Helens South areas. The practices have a combined geographic area made up of around 85,000 patients.
This role will contribute to the improving quality of care of our patients across the PCN and its network of practices.
