Job summary

An opportunity has arisen to join our friendly practice as a Practice Nurse. You will be part of a team which prides itself in providing a high standard of care for our patients.

The ideal candidate will already have all basic nursing skills, including baby immunisations, cytology and chronic disease management including a special interest in diabetes and travel immunisations.

Main duties of the job

Main Duties of the Job to include:

  • Assist in and perform routine tasks related to patient care as directed by GPs
  • Cytology
  • Wound care / ulcer care/ Doppler
  • Removal of Sutures
  • Travel Clinic
  • ECGs
  • Venepuncture
  • Hypertension Management
  • Routine immunisations/Childhood immunisations
  • Requesting pathology tests, for example urine culture, swabs
  • Chronic disease management, particularly Diabetes
  • Following agreed clinical protocols with referral to GPs as appropriate

The Practice is based on the outskirts of Northampton Town Centre, in purpose built premises. We are high achieving, with approximately 7000 patients.

Job responsibilities

Job Description:

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

a. To support the Infection Prevention Control (IPC) lead and to work to the IPC requirements by providing audits, training, support and evidence of compliance to the practice manager

b. To develop, implement and embed an effective care management programme for People with Learning Disabilities.

c. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

d. To develop, implement and embed health promotion and wellbeing programmes with the nursing team.

e. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

f. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health

g. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE

h. As required, to diagnose and manage acute and chronic conditions

i. Provide wound care (ulcer/Doppler, etc.) to patients

j. To review patient medications to enhance compliance

k. To provide guidance to patients in the use of over-the-counter medications regarding side effects and dosages

l. To assist with the provision of travel medicine services

m. To maintain accurate clinical records in conjunction with extant legislation

n. To ensure SNOMED CT codes are used effectively

o. To ensure the appropriate maintenance and use of chronic disease registers

q. To ensure compliance with the practice chaperone policy

r. To assist GPs with minor surgery when required

s. To prioritise health issues and intervene appropriately

t. To support the team in dealing with clinical emergencies

u. To recognise, assess and refer patients presenting with mental health needs

v. To participate in vaccination programmes for adults and children

w. To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)

x. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

y. To support the clinical team with any safeguarding matters, in accordance with local and national policies

z. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
