Clinical Pharmacist - (PCN- Primary Care Network) (ARRS)

Job summary

The Clinical Pharmacist will be employed under the NHS England Additional Roles Reimbursement Scheme (ARRS) to work as part of the multidisciplinary Primary Care Network team. The role supports medicines optimisation, improves patient safety, and enhances access to clinical care, while helping to reduce GP workload.

The post holder will work within their professional scope of practice and competency framework, under appropriate clinical supervision, delivering patient-facing pharmaceutical care across the PCN.

Main duties of the job

  • Conduct structured medication reviews (SMRs) for patients with complex needs, including:
    • Polypharmacy
    • Frailty
    • Long-term conditions
    • Care home residents
  • Provide clinical advice on prescribing, medicines optimisation, and deprescribing.
  • Manage medication-related queries from patients and clinical staff.
  • Support chronic disease management clinics in line with agreed protocols.
  • Undertake medication reconciliation following hospital discharge.
  • Identify and manage medicines-related safety issues and risks.
  • Prescribe medicines independently (where appropriately qualified and authorised).
  • Support safe, effective, and cost-effective prescribing across the PCN.
  • Promote evidence-based prescribing and adherence to local and national guidelines.
  • Support QOF, IIF, and PCN service delivery priorities where appropriate.
  • Contribute to reduction of medicines waste and prescribing variation.
  • Support implementation of prescribing policies and formularies.
  • Work collaboratively with GPs, nurses, paramedics, and other ARRS roles.
  • Participate in MDT meetings, care planning, and PCN clinical discussions.
  • Liaise with community pharmacies, secondary care, and wider healthcare teams.
  • Support education and advice to practice staff on medicines-related issues.

Nexus MK PCN offices are based in Linford Wood Milton Keynes and cover 5 practices which include Oakridge Park Medical Centre, Wolverton Health Centre, The Stonedean Practice, Sovereign Health Centre, Purbeck Medical Centre.

  • Work flexibly across PCN practices in line with service need.
  • Accurately record all patient contacts and interventions on the clinical system - SystmOne.
  • Ensure all activity is coded appropriately to support ARRS reporting and funding assurance.
  • Adhere to PCN and practice policies, including safeguarding, infection control, and information governance.

Clinical Governance & Quality

  • Maintain professional registration with the General Pharmaceutical Council (GPhC).
  • Work within ARRS role scope and PCN governance arrangements.
  • Participate in audit, quality improvement, and service evaluation.
  • Comply with safeguarding, health & safety, and data protection requirements.
  • Participate in incident reporting and reflective learning.

Education, Training & Development

  • Complete and maintain the Primary Care Pharmacy Education Pathway (PCPEP) if required.
  • Maintain ongoing Continuing Professional Development (CPD).
  • Participate in supervision, appraisal, and PCN development programmes.
  • Support training and development of colleagues where appropriate.
