Main duties of the job

As an Advanced Nurse Practitioner, you'll play a key role in delivering high-quality, patient-centred care within a supportive primary care team. You'll work autonomously to assess, diagnose, and treat a wide range of health conditions, including urgent, acute, and chronic presentations. You'll also contribute to improving health outcomes across the community by supporting personalised care, engaging in population health initiatives, and mentoring junior staff. Job responsibilities

Provide expert clinical assessment, diagnosis, treatment, and evaluation of care for patients presenting with undifferentiated and undiagnosed conditions.

Manage long-term conditions (e.g., asthma, diabetes, COPD) and implement individual care plans.

Undertake same-day urgent appointments, triage, and manage minor illness clinics.

Prescribe medications and treatments as an independent nurse prescriber.

Review and action pathology results and hospital correspondence.

Carry out audits, contribute to QI projects, and deliver key components of the PCN DES.

Work collaboratively with GPs, pharmacists, HCAs, social prescribers, and the wider MDT.

Contribute to supervision, mentoring, and education of junior clinical staff.

Champion personalised care and population health management.

Qualifications

Essential

  • Registered Nurse with current NMC registration.
  • MSc in Advanced Clinical Practice (or working towards).
  • Independent Nurse Prescriber qualification.

Experience

Essential

  • Experience working in a primary care or urgent care setting.
  • Management of patients with complex and long-term conditions.
  • Experience of autonomous practice and clinical decision making.

Desirable

  • Experience delivering minor illness and/or same-day access clinics.
  • Familiarity with PCN DES and local health priorities.
