Job summary
We are seeking a motivated and compassionate Practice Nurse to join our multidisciplinary team. You will play a key role in delivering high-quality nursing care to our patient population, supporting both chronic disease management and preventative healthcare.
Main duties of the job
Deliver general nursing services including wound care, immunisations, cervical cytology, and health checks
Support the management of long-term conditions such as diabetes, asthma, and COPD
Provide health promotion and lifestyle advice to patients
Assist in infection prevention and control measures
Maintain accurate clinical records in line with practice policies
Work collaboratively with GPs and the wider healthcare team
About us
St. Andrew's is a large, friendly, very busy practice of 15900 patients. You will work as part of a large, multi-disciplinary team including ANPs, APPs, Physiotherapist, Pharmacists, nurses and health care assistants. We have four GP partners and four salaried GPs and a very friendly admin team supporting everyone.

Undertake chronic disease reviews and monitoring

Provide lifestyle advice (e.g. smoking cessation, weight management, alcohol reduction)
Provide holistic, patient-centred care

Promote health education and self-management

Support patients in understanding and managing their conditions

Act as an advocate for patients where appropriate
Maintain accurate and up-to-date patient records

Work within NMC Code of Conduct and practice policies

Participate in clinical audits and quality improvement initiatives

Ensure infection prevention and control standards are maintained

Safely manage and administer medications (including under PGDs where applicable)
Work collaboratively with GPs, healthcare assistants, pharmacists, and other MDT members

Support and mentor junior staff or students where appropriate
