Job summary

In this role, you will work under the guidance of the Partners and Clinical Pharmacist to ensure the accurate, timely, and safe supply of prescribed medications.

Main duties of the job

You will provide both technical and administrative support to clinicians and assist with patient medication queries.

Job description

Job responsibilities

  • Undertake patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients
  • High risk drug monitoring
  • Conduct medicines optimisation tasks including effective medicine administration ,support medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.

  • Perform medication switches or dose adjustments as directed by prescribers or pharmacists.
  • Assist with medication queries from patients, community pharmacies, and prescribers, and forward recommendations for alternatives.
  • Support implementation of MHRA alerts, safety updates, and prescribing audits.
  • Utilise the GP practice clinical systems (EMIS Web) to conduct prescribing audits.
  • Support QOF and CQC searches related to prescribing safety.

  • Qualified Pharmacy Technician registered with the General Pharmaceutical Council (GPhC).

Desirable

  • Enrolled in, undertaking or qualified from, an approved training pathway. For example, the Primary Care Pharmacy Educational Pathway (PCPEP)

  • Experience of working in a primary care setting
