The successful candidate is someone who is motivated and committed to providing high quality care, working collaboratively with the general practice team to meet the needs of our patients.
Previous experience of working in general practice is essential.
Main duties of the job
Management of chronic disease -monitoring and development of individual care plans, agreeing these as appropriate with the patient and other health professionals .
Family planning & woman's health including cervical smears.
Travel health advice and vaccinations.
Child immunisations and advice.
Deliver local and national vaccination programmes
QOF, DES & LES Implementation.
CVD Clear programme monitoring.
Assist in the provision of minor surgery and family planning clinics. (sickness/annual leave cover).
