Main duties of the job
As an Advanced Nurse Practitioner at Swan Medical Centre, you will be responsible for delivering high-quality, evidence-based clinical care to patients across a wide range of acute, chronic, and complex presentations. Working autonomously within your scope of practice, you will assess, diagnose, treat, and manage patients, including independent prescribing, ordering and interpreting investigations, and making appropriate referrals to other services.
The role requires confident clinical decision-making, effective risk management, and a commitment to maintaining patient safety and professional standards. You will work closely with GPs, nurses, and the wider multidisciplinary team to ensure continuity of care, contribute to shared clinical decision-making, and support service development within the practice.
A strong patient-centred approach is essential. You will build trusted relationships with patients, communicate clearly and compassionately, and involve individuals in decisions about their care. You will play an active role in promoting health, supporting self-management, and delivering a positive patient experience while helping the practice meet quality and regulatory expectations.
