Job summary
PCN Clinical Pharmacist
This role is pivotal to improving quality of care and operational efficiencies which requires motivation and passion to deliver an excellent service.
The role will involve working within the Primary Care Network and our practices.
If you hold an MPharm qualification and have registration with the GPhC this could be the role for you.
Applicants will need to complete the CPPE pathway - 18 months, which you would be enrolled on once in post.
Main duties of the job
The role involves supporting general practice staff with prescription and medication-related queries. Key responsibilities include managing the repeat prescription process, handling acute prescription requests, carrying out medicines reconciliation after care transitions, and promoting safer prescribing. The pharmacist will provide expert clinical advice while taking into account both the healthcare and social care needs of patients across the practice.
