Job summary

Bordesley East Primary Care Network (PCN) would like to recruit a forward thinking and enthusiastic Care Coordinator to support the practices in expanding services available to our patients.

This is a very exciting opportunity to get involved in making a real difference within primary care and supporting our patients.

The practices are a supportive and friendly group whose aim is to provide high quality, seamless health care that enables people to lead healthier lives, whilst feeling supported and cared for.

Main duties of the job

This role will focus on but will not be limited to:

1. Supporting the PCN to achieve goals within the Investment and Impact Fund (IIF) and the PCN Directed Enhanced Services (PCN DES).

2. Working closely with GPs and other primary care professionals within the PCN.

3. With support focusing on the delivery of a comprehensive model to reflect local priorities, health inequalities and population health management.

The PCN is made up of 5 member GP practices serving over 30,000 patients. We offer services ranging from Social Prescribing, Physiotherapists, Paramedics and Clinical Pharmacists to make a range of services more accessible to our patients.

The role will support practices and patients in receiving the right care the first time around.

  • Able to use Microsoft Office applications with above average capability with Word, Excel and PowerPoint.

  • A good level of education (5+C GCSEs)
  • Equivalent Experience
  • Educated to A level in relevant subject or equivalent level (Desirable)
