Job summary
The Lead Practice Nurse provides clinical leadership and management to the nursing team including HCA's, ensuring the delivery of safe, effective, and evidence-based nursing care within the practice. This role involves coordinating and overseeing nursing services with the admin team, supporting chronic disease management implementation, ensuring that clinical policies and protocols are adhered to, and contributing to the overall quality improvement of patient care.
Your role will be split between leadership time and face to face clinical time with patients.
Main duties of the job
Leadership - to provide professional leadership to ensure the delivery of safe, high quality , effective and compassionate care. To up hold standards of the profession in accordance with NMC code and to challenge poor practice. To be an expert resource for nursing colleague, provide support and governance and uphold practice principles. To work with the practice manager in performance related issues.
Work alongside senior management colleagues to maintain Practice goals and standards whilst ensuring nursing team are compliant with Practice protocols.
Clinical time - You will provide general nursing duties , including all usual treatment room services (childhood immunisations, cytology, bps and blood taking, travel advice etc ) and have a broad range of skills with long term condition care of a varied nature. An interest/ additional qualification in heart disease and/or diabetes would be beneficial to the role but not essential.
Use of EMIS computer system a significant advantage.
About us
Cantilupe and Hampton Dene surgeries are based in Hereford city. We serve a population of 12600 patients within a 3 mile radius of the surgery.
The practice has 3 GP Partners, 4 Salaried GPS, 3 ANPS, 1 Prescribing Paramedic, a team of 4 other Practices Nurses and 3 HCAs.
They are supported by a large reception and admin team.
Our QOF rating is good' and we strive to provide a good service for our patients at all times.
