Job summary

You will be a degree qualified Advanced Nurse Practitioner with relevant clinical skills and expertise to work autonomously. You must have relevant experience to be an independent nurse prescriber enabling you to independently assess, diagnose, treat, and refer patients as appropriate. A specialist area of interest would be an advantage. We are supportive of training and development and welcome new innovative ideas.

Main duties of the job

  • Work independently, making professional, autonomous decisions.
  • Demonstrate a broad and sound knowledge of complex and common conditions in primary care in both adults and children
  • History taking, clinical assessment, diagnosis, treatment plan and evaluation of patient care.
  • Referring to secondary care, where appropriate.
  • Demonstrate safe clinical decision making and a high level of independent care including assessment, diagnostic and prescribing skills.
  • Prioritise and triage the needs of patients, accordingly, seeking GP input as appropriate where the problem lies outside their professional capability.

About us

Great Homer Street medical Centre is based in a fresh, modern building and we pride ourselves on being friendly and welcoming. A small/medium sized practice with a supportive clinical team consisting of a GP partner, salaried GP, long term locum GP, 1 x practice nurse, 1 x HCA and an admin team whose priority is to provide good high-quality care to our patients.

We are a training practice training future GPs and medical students and are supportive of training and development for our team.

We have access and support from a PCN ARRs team consisting of pharmacists, paramedics, mental health practitioners, care co-ordinators, social prescribers, occupational health therapists and physiotherapists.

The practice has extensive community links and has been involved in several inequity project in recent months.

Key Responsibilities:

1. Clinical Assessment and Diagnosis:

Independently assess and diagnose patients presenting with a wide range of acute and chronic conditions.

Order and interpret diagnostic tests, including laboratory work, radiology, and other relevant investigations.

Develop differential diagnoses and treatment plans based on evidence-based guidelines and clinical expertise.

2. Prescribing and Medication Management:

Prescribe medications, treatments, and therapies as appropriate to patients' conditions and needs.

Monitor medication effectiveness and potential adverse reactions, adjusting as necessary.

Provide education to patients about prescribed medications and their proper administration.

3. Health Promotion and Education:

Conduct health promotion activities within the practice and the community to address prevalent health issues and raise awareness.
