- Provide the Home Visit
service for the practice.
- Assess, diagnose, and
develop effective clinical management plans.
- Provide face-to-face or
telephone consultations for undifferentiated patient presentations.
- Manage acute and chronic
conditions with confidence and compassion.
- Offer patient education and
support, promoting self-management and healthy choices.
- Contribute to the delivery
of practice policies and procedures.
