Main duties will be to complete Chronic disease reviews, medication reviews and see patients who have a minor illness.

Conduct comprehensive medication reviews to optimize patient treatment plans.Review and manage acute medication requests with a focus on patient safety and efficacy.Efficiently process hospital discharge summaries and clinic letters, ensuring seamless patient care transitions.Offer expert support and guidance for patients managing self-limiting conditions, empowering self-care.Support practices in achieving Quality Outcomes Framework (QOF) targets, enhancing practice performance and patient outcomes.Play a pivotal role in enhancing health outcomes for patients with long-term conditions through collaborative care.Undertake additional tasks as needed to support the healthcare team and patient care initiatives.

Experience

Essential

  • Minimum of 2 years experience in a GP Practice.
  • GPhC registration.
  • Independent Prescriber Status (preferred).
  • Proficiency in EMIS.

Desirable

  • Experience in running chronic condition clinics.
  • Proficient in structured medication reviews and acute medicines request reviews.
