Job summary

We are recruiting an ACP to work 2 or 3 days per week and join our multi-disciplinary clinical team providing a high standard of care for our practice population of approx 12,900 patients.

We are a forward thinking practice with a friendly, approachable and supportive team. We are a training practice, with much experience of training staff in a variety of our valued roles and supporting personal development.

The surgery is in a modern purpose built building with free staff parking and is close to good public transport links.

Main duties of the job

The successful applicant will undertake a variety of duties, including surgery consultations, telephone and online consultations and queries, visiting patients at home, dealing with queries and clinical admin work in a timely fashion.

Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation

Assessing the health care needs of patients with undifferentiated and undiagnosed problems

Screening patients for disease risk factors and early signs of illness

Developing care plans for health in consultation with patients and in line with current practice disease management protocols

Providing counselling and health education

Admitting or discharging patients and referring to other care providers as appropriate

Recording clear and contemporaneous consultation notes to agreed standards

Supporting other clinical colleagues

Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)

Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate

About us

Our practice team is led by 4 partners and comprises of GP's, GP reg, Salaried GP, ANP, ACP's and Trainee ACP, Physician Assistants, Practice Nurses, Nurse Associates, Healthcare Assistants, Pharmacist, Pharmacy Technicians and GP Assistants.These are all well supported by our reception and admin team.

We have a close practice team of over 40 staff who are friendly, approachable and supportive. We strive to provide a high standard of care for our patients with the resources available to us and working in partnership with wider health and social care services.

The practice is an active member of Dewsbury & Thornhill PCN and Curo Healthcare GP Federation working with other local practices to develop population care.

Job description

Job responsibilities

The following are the core responsibilities of the advanced clinical practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a.Develop, implement and embed health promotion and well-being programmes

b.Manage and treat patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients in practice or in their homes for housebound patients

c.Implement and evaluate individual specialised treatment plans for chronic disease patients

d.Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

e.Provide advanced, specialist care to patients as required in accordance with clinical based evidence, NICE and the NSF

f.Request pathology services as necessary

g.Process and interpret pathology and other test results as required

h.Maintain accurate clinical records in conjunction with extant legislation

i.Ensure read codes are used effectively

j.Maintain chronic disease registers

k.Chaperone patients where necessary

l.Prioritise health issues and intervene appropriately

m.Support the team in dealing with clinical emergencies

n.Recognise, assess and refer patients presenting with mental health needs

o.Extended and supplementary prescriber, adhering to extant guidance

p.Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

q.Contribute to practice targets (QOF etc.), complying with local and regional guidance

r.Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

s.Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

t.Support the clinical team with all safeguarding matters, in accordance with local and national policies

u.Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

v.Deliver opportunistic health promotion where appropriate
