Job responsibilities
General:
To provide general medical services to the registered
practicepopulation and temporary patients in accordance with theGMS/PMS terms of service, managing a caseload and
dealing witha wide range of health
needs in a primary care setting, ensuringthe highest standards of care.
To develop primary care
services, working closely with thepractice team,
local population, the Local Presence, PCN,community field staff, and other PMS/GMS GPs to establishneeds and
appropriate service requirements.
To work with clinical colleagues within the practice to
ensure thatthe full benefits of medical services are realised.
The post
involves supporting the work of the practices in managing a busy and
interesting clinical caseload. The
appointee will take part in leading on key disease
areas (monitoring trends in treatment, informing on best practice, staff
training and prescribing) and assist in developing appropriate healthcare services for a diverse,
patient list.
CLINICAL DUTIES
To provide
general medical services
to the registered populations of the two practices and to
ensure that targets are met in terms of activity and quality in line with the
new GMS contract and the
PMS contract and any local or
national incentives including
QoF, Enhanced Services, etc.
In accordance
with the practice timetable, to make him/her-self available to undertake a variety of duties including surgery
consultations and emergencies, telephone/online consultations and queries,
triaging, visiting patients
at home; checking and signing repeat prescriptions including electronic prescriptions, dealing with queries,test results,
patient medical reports, making
appropriate referrals within
the local guidelines; dealing with any other paperwork and correspondence in a timely fashion.
Making
professional, autonomous decisions in relation to presenting problems. Recording clear and contemporaneous consultation notes to agreed standards using Emis Web.
To screen patients
for disease risk factors and early signs of illness.
In
consultation with patients and in line with current disease management
protocols, to develop care plans for health.
To provide counselling and health education.
To participate in shared care arrangements with colleagues within the local
and other acute
trusts, other General
Practice colleagues and community nursing
and professions supplementary to medicine.
To
contribute to the updating and development of clinical policies, and contribute
to borough-based / practice led projects in order to develop the services offered
to patients.
To
be active in clinical audit and research, and participate in the practices
clinical governance programmes.
Compiling and issuing computer
generated acute and repeat prescriptions.
To adhere to data quality (clinical
coding / templates) guidelines and the practice prescribing policies.
To be aware of the Directed,
National and Local Enhanced Services
and KPIs commissioned from the practices and to
deliver these in accordance with the published
criteria.
To
act quickly to protect patients from risk if the post holder has good reason to
believe he/she or a colleague
may not be fit to practice by using the practice reporting system.
In
general, the post-holder will undertake all the normal duties andresponsibilities associated with a GP working in primary care.
SERVICE MANAGEMENT
Support and motivate staff within the Primary Health Care Team in providing
medical services and meeting
targets.
Ccollaborate with community-nursing colleagues in the Primary Health Care Team to ensure that the skills of members are utilised effectively and that work is
delegated appropriately.
Work with clinical and other colleagues within the practice and other agencies
to ensure
that appropriate and efficient services are provided within contracted
resources and that the local populations health needs and service development needs are identified.
Participate in special developments/projects in line with practice service
objectives.
Participate in the implementation of Information Systems
for the practice
COMMUNICATION
The post holder should
recognise the importance of effective communication within the team and strive to communicate
effectively with colleagues, patients and carers, andrecognise the need for alternative methods
of communication and respond accordingly.
QUALITY
The
post holder will strive to maintain quality within the practice, and against
agreed standards, and take
corrective action where necessary.
Develop audit and to contribute to multi-disciplinary audit and the practices
clinical governance programme, as appropriate.
Develop ways of involving the patient in discussions and decisions, which seek
to ensure that we provide appropriate and efficient services.
Contribute to the ongoing
practice quality, improvement and risk management programme, including participation in local quality improvement (QI) and risk management activities.
Be responsible for identifying the need for patient information and to
contribute to its production.
Participate in and cooperate with annual appraisal and assessments, in line
with NHS policy.
Contribute to Continuous Professional Development and updating skills and knowledge
as identified through
review and appraisal.
Alert
other team members to issues of Clinical Governance issues, quality and risk; participate in Significant Event Analysis
reviews.
Assess
own performance and take accountability for own actions, either directly or
under supervision.
Contribute
to the effectiveness of the team by reflecting on own and team activities and making suggestions
on ways to improve and enhance the teams
performance.
Work effectively with individuals in other agencies
to meet patients needs.
Effectively
manage own time, workload and resources. He/she will also contribute to the overall
team working of the Practice putting the needs of the Practice
first.
OTHER
Teaching
and training of doctors, nurses, trainees
and students.
Awareness of and compliance with all relevant
practice policies/guidelines, e.g. prescribing,
confidentiality, data protection, health and safety, clinical and information governance and be involved in the annual
Assessment of the Practice and in the annual contract
review process and CQC
Make a commitment to life-long learning
and professional development to ensure high quality, evidence-based best practice.
Maintain a PDP/learning portfolio
Contribute to evaluation, audit
and clinical standard
setting within the organisation.
Attending training, practice meetings and events
organised by the practice or other agencies, where appropriate.
Meet all timescales/deadlines for reports and
written returns to ensure that the practice
meets quality and activity standards (e.g. Enhanced Services returns, annual QOF audit etc.)
