Job summary
We are looking for a compassionate and skilled Practice Nurse to join our dedicated primary care team. This role has become available as we say goodbye to a valued Practice Nurse who has served our patients and is relocating to another practice.
In this role, you will provide a range of nursing services to support the health and wellbeing of our patient population. You will work closely with GPs, other nurses, and healthcare professionals to deliver high-quality care, including managing long-term conditions, administering immunisations, and supporting health promotion initiatives.
You will be an integral part of a multidisciplinary team, creating a welcoming and non-judgmental environment for patients, while upholding the values of care, compassion, competence, communication, courage, and commitment. This role offers the opportunity to develop your professional skills, contribute to patient care planning, and actively participate in the ongoing development of the nursing team.
Main duties of the job
The Practice Nurse provides essential nursing care to meet the needs of our patient population, including managing long-term conditions, delivering immunisations and screening programmes, and promoting overall health and wellbeing. A key part of the role is helping patients take control of their own health and working collaboratively to improve the health of the practice population.
You will be part of a multidisciplinary team of GPs, nurses, and administrative staff, providing care in a welcoming, non-judgmental environment. Duties include delivering nursing treatments independently or with GPs, reviewing test results, performing home visits, supporting patient recalls, and maintaining accurate documentation.
The role also involves supporting the nursing team, assisting junior staff, contributing to training, maintaining stock, and ensuring safe use of equipment. You will follow all relevant policies and protocols, including health and safety, infection control, equality, and confidentiality, while communicating effectively with colleagues and external agencies.
Professional development is encouraged, with accountability for your own clinical practice, ongoing training, and preparation for NMC revalidation. This is a varied and rewarding role offering the chance to provide high-quality patient care while supporting a patient-focused practice team.

- Ensure accurate completion of all necessary documentation associated with a patients healthcare.
- Perform home visits where appropriate.
