About
the Role
We
are seeking a caring, organised and motivated Care Coordinator to join our
friendly GP practice team. This role combines personalised care and patient navigation
responsibilities with essential front of house and administrative duties,
supporting the smooth and effective operation of the practice.
You
will play a key role in ensuring that patients particularly those who are
frail, elderly, or living with long term conditions receive joined up, person centred
support. As an integral member of our reception and administrative team, you
will help patients access the right care at the right time while maintaining
high standards of service and professionalism.
The
postholder will work with a diverse range of people from a variety of cultural
and social backgrounds. The ability to work confidently, sensitively and
effectively in a varied and sometimes demanding environment is essential.
The
successful candidate will have excellent communication and interpersonal
skills, be patient, empathetic and highly organised, and will bring experience
of working in health, social care or other support roles involving direct
contact with individuals, families or carers.
Main duties of the job
Key Responsibilities
Personalised Care & Care Coordination
- Proactively identify and work with people who would benefit from care coordination, including those with complex needs.
- Support patients to develop and review their personalised care and support plans.
- Act as a central point of contact, helping patients navigate health, social care and community services.
- Liaise closely with GPs, nurses, social prescribers, health and wellbeing coaches and other MDT members.
- Assist with follow up after hospital care, transitions between services, and ongoing communication.
Reception & Front of House Duties
- Provide a welcoming, professional reception service to all patients and visitors.
- Book, amend and triage appointments (telephone, facetoface and online).
- Process home visit requests, clinical messages and signposting queries.
- Handle incoming calls, emails, online requests and post.
- Support with test result queries and specimen handling in line with protocols.
Administration & Data Quality
- Register new and temporary patients and update patient information.
- Process repeat prescriptions following practice guidelines.
- Scan, index and code clinical documents using appropriate SNOMED codes.
- Maintain high quality record keeping and support data collection for audits/service improvement.

Helix Medical Centre is based within Herts and Essex Community Hospital in Bishop's Stortford. We are members of the local Primary Care Network providing extended services as a hub to the local community. We also work with Allied Health professionals together with an expanding complement of ARRs staff including Physicians Assistant, Clinical Pharmacist, Community Paramedic, Community Midwife, Health and Wellbeing Advisor, Care Co-ordinator, Social Prescribers and First contact physio, Dietician etc. There is a wide socio-economic and demographic served within our catchment area providing an interesting and varied case mix.

Hours of work: 38 hours per week as follows:
Monday 8am - 6.30pm
Wednesday 8am - 6.30pm
Thursday 8am - 6.30pm
Friday 8am - 6.30pm
Rate of Pay: £12.21 per hour, rising to £12.71 as of 01/04/26
Annual Leave: 5.6 weeks annual leave inclusive of bank holidays. Entitlements are pro-rata for part-time employees.
Pension: Option to join NHS Pension scheme.
