Job summary
Birchwood Medical Centre has an exciting opportunity for an
enthusiastic and motivated practice nurse to join our dedicated team
of 2 practice nurses, 1 nursing associate and 2 HCAs/phlebotomists. The post is for 16 hours per week, Monday and Tuesday, 9.00 am to 5.30 pm.
Main duties of the job
The successful candidate should be experienced in
primary care with key skills to include chronic disease management (including diabetes,
asthma, COPD), childhood immunisations, travel vaccinations, cervical cytology,
OQF and other routine practice nurse duties such as BP checks, dressings, ear
syringing and ECGs, although training opportunities may be available for the
right candidate. You will provide high
quality care to all our registered patients, working within your own competence
and professional code of conduct as regulated by the NMC. Consultations may be face to face or via telephone as appropriate.
About us
We are a friendly, supportive training practice of 11,200
patients and operate from well-equipped, purpose-built premises on the outskirts
of Warrington. We comprise of 3 Partners, 7 salaried GPs, 1 nurse clinician, 1
GP Registrar, 1 FY2 and medical students. We have a supportive reception and admin team, as well as an expanding compliment of ARRs staff including a clinical pharmacist, first contact practitioners, a mental health practitioner, a care co-ordinator and a social prescribing link worker. We are located close to junctions on both the
M6 and M62 and the train station is just a few minutes walk away. We are actively involved with the East Warrington
PCN, which comprises 2 other local practices. We are CQC rated good and have a consistently high QOF achievement.

- To work as part of the primary healthcare/nursing team with the responsibility for delivering safe and effective nursing care, with a focus on delivering evidence based practice for patients with long term conditions and management and nursing interventions
- Implement and evaluate individual treatment plans for patients with chronic diseases
- Identify, manage and support patients at risk of developing long term conditions
- Maintain robust call and recall systems
- Offer
a holistic approach to travel health by providing comprehensive advice to
patients prior to travel including vaccinations and medicines
- Offer
advice about childhood and adult vaccinations and ensure vaccines are
administered under patient group directions
- Assessing
problems presented opportunistically by patients, dealing with minor
illness.
- Able
to document consultations according to NMC guidelines
- Ensures
awareness of statutory and local clinical protection procedures, including
systems of referral. Ability to
recognise signs and symptoms of child abuse.
- Perform
a holistic assessment of patients attending for cervical cytology smear tests.
- Perform
venepuncture according to local guidelines
- Provide
a holistic approach to wound management and implement wound care in line
with current evidence based guidelines
- Able
to recognize and manage anaphylaxis according to current UK guidelines

to perform Cardio-pulmonary resuscitation according to current UK
guidelines

to obtain and document informed consent (either verbal or written)

infection control guidelines are maintained
