Harwood Group Practice is excited to invite an Advanced Nurse Practitioner to join our friendly and forward-thinking team.
The RoleThis is a full-time position (37.5 hours per week) operating between 08:00 18:30, Monday to Friday.
As a key member of our clinical team, your time will be split between providing high-quality care within the practice and conducting home visits for our more vulnerable patients in the community who are unable to attend the surgery.
Key ResponsibilitiesAutonomous Care: Assess, diagnose, and treat patients with a wide range of acute and chronic conditions.
Community Outreach: Provide essential clinical support through home visits, ensuring continuity of care for housebound patients.
MDT Collaboration: Work closely as part of a diverse Multi-Disciplinary Team (MDT), including GPs, nurses, and allied health professionals, to deliver integrated patient care.
Clinical Excellence: Maintain high standards of prescribing and clinical decision-making to improve patient outcomes.
Candidate Requirement: The successful applicant must be a qualified Advanced Nurse Practitioner with independent prescribing rights and a passion for both practice-based and community medicine.
Main duties of the job
Main Duties and Daily Responsibilities:
Assessing and treating "same-day" urgent conditions such as respiratory infections, abdominal pain, skin infections, and minor injuries.
Chronic Disease Management: Proactively managing long-term conditions, adjusting medication regimes, and preventing hospital admissions.
Daily Tasks and Responsibilities
Autonomous Consultations: Conducting face-to-face, telephone, and video consultations to triage, assess, and diagnose patients.
Physical Examinations: Performing comprehensive physical examinations.
Independent Prescribing: Issuing prescriptions safely and effectively in accordance with your scope of practice and local formularies.
Diagnostic Requests: Ordering and interpreting clinical tests, including bloods, X-rays, and ultrasounds, and acting promptly on the results.
Referrals & Signposting: Making appropriate referrals to secondary care (hospitals) or specialist community services (e.g., District Nursing, Palliative Care).
MDT Participation: Attending daily clinical "huddles" or MDT meetings to discuss complex cases, safeguarding concerns, and end-of-life care.
Clinical Administration: Managing your own clinical workflow, including processing clinical letters, actioning lab results, and maintaining accurate, contemporaneous records via EMIS.
Health Promotion: Providing opportunistic screening and health advice, ensuring patients are empowered to manage their own wellbeing.

Job SummaryThe post-holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
The role is split between site-based clinics and a community-based home visiting service, ensuring that patients who are housebound or in residential care receive high-standard clinical intervention. As an independent prescriber, you will be responsible for the complete episode of care, from diagnosis to issuing necessary prescriptions.
Main Duties & Responsibilities1. Clinical Practice & PrescribingAutonomous Consultations: Deliver face-to-face and telephone consultations for patients with undifferentiated and undiagnosed problems.
