Job summary

We are seeking an experienced Advanced Nurse Practitioner to join our friendly team. The successful candidate should be able to assess, diagnose, treat, and manage a wide range of health conditions. You should be confident to work autonomously and provide a high level of patient care. In this role, you will be a vital member of our team, providing expert care in the practice & on home visits.

Main duties of the job

The main duties for this role include:

  • Seeing and treating acutely unwell patients to provide immediate, high-quality care.

  • Managing chronic illnesses, providing ongoing support and treatment to patients with long-term conditions.

  • Providing a home visiting service for acutely unwell patients who are unable to attend the practice.

  • Working alongside trainees to mentor and support the next generation of healthcare professionals.

  • Supporting the GP Partners in delivering excellent patient care and contributing to the overall success of the practice.

About us

Our Practice has a large, multi-disciplinary team, with six GP Partners, two Salaried GPs, an Advanced Nurse Practitioner, an Advanced Clinical Practitioner, a Paramedic, and five Pharmacists. We also have two MSK Practitioners, four Practice Nurses, a Health Care Assistant, a Nurse Associate, two HIPS, and a large administrative team that keeps everything running smoothly.

This mix of skills and experience creates a lively and inclusive atmosphere where you'll be part of a team. We work together across two sites, in Harwood and Tonge Moor, and are proud members of the successful Turton PCN, which further expands our network and opportunities.

The Role:

We are seeking an enthusiastic and experienced Advanced Nurse Practitioner to join our expanding team. This is a crucial role that will involve working autonomously and as part of a multidisciplinary team to deliver comprehensive primary care services. The successful candidate will be a key player in supporting the partners to achieve our Quality and Outcomes Framework (QOF) and Bolton Quality Contract (BQC) targets.

Key Responsibilities:

  • Clinical Practice:

    • Conducting autonomous clinics, assessing, diagnosing, treating, and managing a wide range of acute and chronic illnesses in patients of all ages.

    • Prescribing and reviewing medications in line with current NICE guidelines and local formularies.
