Main duties of the job
Job Responsibilities
To deliver practice nursing services within NMC Code of Conduct and agreed Practice standards.
Manage patients with chronic diseases, following quality frameworks and evidence-based guidelines.
Contribute to the development of integrated services within Primary Care.
Oversee infection control, ensuring compliance, and participate in annual appraisals.
Perform clinical duties, including chronic disease management, cervical screening, immunisations, and lifestyle advice.
Recognise signs of abuse and work with safeguarding teams where relevant.
Maintain accurate records, participate in audits, and update disease registers.
Key Skills
Ability to work independently and collaboratively within a multidisciplinary team.
Strong organisational and planning skills, able to work under pressure.
Excellent communication skills and experience in dealing with the public/patients.

About us
Our well established and well respected practice is currently running across three sites.
The successful candidate will be expected to work between our three sites which are: Elizabeth Street Surgery, Gorton Street Surgery and Harris Medical Centre. We are temporarily working from Elizabeth Street and Gorton Street until our new build on Adelaide Street is ready.
We are a large practice with over 15,000 patients, supported by a very experienced, hardworking and dedicated team.
The team includes doctors, advanced nurse practitioners, pharmacists, pharmacy technicians, physician associates, physiotherapists, care co-ordinators, social prescribers, practice nurses, paramedics, mental health practitioner's, health and wellbeing coach's, nurse associate's, assistant practitioner's, health care assistant's and a large reception and administrative team.
We pride ourselves on being a training practice supporting GP trainees, medical students etc.

Delivery effective implementation of management of patients with Chronic Diseases as detailed by the Quality and Outcomes Framework, local quality schemes and national quality schemes
Support the implementation of evidence-based guidelines in conjunction with the wider team.
Ensure that all nurse-related Care Quality Commission evidence table indicators are compliant, demonstrating evidence, review, and audit cycle
Participate and engage in the practice appraisal process working towards a personal development plan, utilising a reflective approach to practice.
Actively participate in multi-disciplinary clinical and practice meetings.
Support and participate in the development of clinical and practice protocols.
A range of clinical nursing duties in line with the practice nursing team.

Provide adult and child immunisations
Ensure safe storage, rotation, and disposal of vaccines and drugs
Undertake venepuncture and electrocardiography

Deliver travel health care, including vaccinations and advice for patients
Provide high quality evidence-based care.
Undertake a variety oof research and analysis tasks associated with the improvement of clinical care, medical diagnosis and treatment.
Maintain and update disease registers.
Promote and support a learning culture within the Practice and assist in clinical instruction, mentoring and supervision of trainees attached to the practice.
