Oak Tree Surgery and Pensilva Health Centre are seeking a motivated and enthusiasticGeneral Practice Nurse with Diabetes Interestto join our expanding friendly and dynamic team on a permanent basis. Monday - Thursday 8am - 6pm (36 hours per week).
We are a large successful training practice across two dispensing sites with a practice owned on site Pharmacy and an award-winning research team.

Main duties of the job
Ensure the delivery of safe and effective nursing care to the practice population. A key aspect of the role is the delivery of evidence-based practice for patients with diabetes and pre-diabetes conditions and management and preventative nursing interventions to all patients, including other General Practice Nursing skills. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making. They will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
Your key responsibility is to provide and maintain a high standard of nursing care for our patients as well as providing nursing assistance to the health care team. The Practice population has a high prevalence of chronic diseases, therefore the ability to work under pressure, prioritise, and work flexibly as a part of a team whilst remaining resilient is essential.
Additional responsibility of Infection Prevention and Control (IPC) Lead foe the practice. The successful candidate will oversee the development, implementation, and regular review of infection control policies and procedures in line with national guidelines, ensuring full compliance with regulatory standards. They will provide staff training, conduct audits, monitor incidents, and promote best practice to maintain a safe environment for patients, staff, and visitors.

chronic case load, dealing with presenting patients needs in a primary care
