Job summary
Oak Tree Surgery and Pensilva Health Centre are seeking a motivated and enthusiastic Care Navigator (receptionist) to join our expanding friendly and dynamic team on a permanent basis.
Working hours are 8am - 6pm to cover four days per week (Monday - Thursday) . Successful candidate will be required to travel to both Oak Tree and Pensilva sites.

Main duties of the job
- To be the primary contact for patients
- To ensure patients attend appointments when called by the GP/Nurse
- To treat patients at all times in a courteous and helpful manner, dealing with queries and providing assistance as required
- Ensure reception area is clean and tidy and equipment is working to maintain a professional image.
- To answer the telephone and make required calls in a polite and professional manner
- To process online patient triage requests following practice procedure
- Candidates should be computer literate and possess excellent communication skills.
- To assist with the sorting of patient notes and changing of personal details to maintain efficient records
- To be aware of and use the required forms. Complete all necessary paperwork to maintain efficient records
- To assist with general administration
- To read the staff noticeboard and emails received to keep up to date with activities
- To attend staff meetings. Contribute positively with ideas to improve the overall success of the practice
- Management of lost property complying with all regulations and the Data Protection Act
- To promote a friendly and enjoyable working environment
- To work to the set assessment criteria and objectives as set out in the review system
- To maintain confidentiality at all times
- To report any accidents/incidents to the Practice Manager
