Job summary

We are looking to recruit a motivated, caring and enthusiastic Practice Nurse with excellent communication and interpersonal skills to join our friendly and supportive nursing team, 20-28 hours a week.

You will be responsible for ensuring the delivery of safe and effective nursing care to the whole practice population, working closely with the wider Practice Team to deliver the practice priorities.

Main duties of the job

Clinically, the focus of the role is management of patients with long-term conditions, treatment room duties and general nursing interventions for all patients. It is desirable you have knowledge of Diabetes Management, Asthma/COPD management, Hypertension, Family Planning/contraception, cervical cytology, long term condition reviews, wound care, childhood immunisations and vaccines.

We have 3 Partners, 7 Salaried GPs, 1 Advanced Paramedic Practitioner, Practice Nurses, HCA and Clinical Pharmacists. List size 15,700. EMIS Web, AccuRX & Docman with Workflow Optimisation. We are part of Farnham Integrated Care Services working in collaboration with local practices offering same day access for urgent care along with MSK Practitioners and Home Visiting Services. X-ray, ultrasound and phlebotomy services available within Farnham Centre for Health. We are also a Training Practice. CQC rated 'good' in November 2019.

Job responsibilities

Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being Implement and evaluate individual treatment plans for patients with a known long-term condition

Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care

Deliver opportunistic health promotion using opportunities such as new-patient questionnaire and health checks to assess patients alcohol consumption and invite in accordingly

Assess and care for patients presenting with uncomplicated wounds

Support and advise women requesting information relating to family planning needs

Support and manage health needs of women presenting for cervical cytology consultations

Undertake ECGs.

Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health

Implement and participate in vaccination and immunisation programmes for both adults and children

Advise, support and, where appropriate, administer vaccinations for patients travelling abroad

Organise and run chronic disease management clinics populated with patients from the Practices' clinical registers in Asthma, CHD, COPD, Diabetes, Hypertension, Mental Health and other clinical areas as advised from time to time. Clinics are to adhere to guidance issued by the Practice from time to time, comply with NICE Guidelines and will require data recording using the clinical system protocols.

  • Either a recognised nursing diploma or BSc Current registration with the NMC and RCN.

Desirable

  • Recognised and accredited training programmes in Liquid Based Cytology and Family Planning, Warwick Diploma in Diabetic Care or other diabetic qualification Asthma/COPD recognised qualification
