Job summary
We are seeking an experienced and highly motivated
Advanced Paramedic Practitioner (APP)to join our multidisciplinary team
at Farnham Park Health Group. The role will focus on providing high-quality
patient care by supporting clinical staff in managing acute and chronic
conditions, undertaking home visits, and contributing to Quality and Outcomes
Framework (QOF) achievement work. The successful candidate will play a key role
in reviewing blood test results, prescribing medications within their
competency, and supporting administrative functions to ensure effective patient
management.

practice is essential.
To support the management team in the reviewing of clinical policy and
procedure, including support to more senior members of the clinical team.

An opportunity has arisen for an Advanced Paramedic Practitioner to join our team in the Georgian market town of Farnham with good local
schools and excellent transport links to London. The surgery is located within
a modern purpose-built building with free parking. We are a forward-thinking
practice, comprising of 4 partners, 6
salaried GPs, 1 advanced paramedic practitioner, practice nurses, clinical
pharmacists, pharmacy technicians, and 4 care coordinators. List size
16,500. We work collaboratively with
other Farnham practices both within our PCN and as part of Farnham Integrated
Care Services offering same day access appointments, MSK and mental health
practitioners, home visiting paramedics and social prescribers. X-ray walk -in
service, ultrasound and phlebotomy available within Farnham Centre for Health.

Job responsibilities
- Reviewing and Actioning Blood Test Results:
- Request pathology and analyse blood test results and discuss management plans with patients.
- Liaise with GPs and other healthcare professionals to ensure appropriate follow-up.
- Home Visits for Housebound Patients:
- Conduct home visits to assess and manage chronic health conditions for housebound patients.
- Perform clinical assessments, initiate treatment plans, and escalate care where necessary.
Supporting QOF Achievement Work:
- Contribute to QOF-related consultations via face-to-face and telephone appointments.
- Assist in chronic disease management and maintaining of registers, including monitoring and reviewing patients.
Urgent Care Support & Duty Doctor Assistance:
- Support the Duty Doctorin triaging and managing urgent, same-day patient cases.
- Conduct face-to-face and telephone triage consultations, providing urgent treatment as required.
Prescribing Medications (Within Competency):
- Prescribe medications where appropriate and within the scope of practice.
- Provide medication reviews and adjust treatment plans accordingly.
Administrative & Team Support:
- Document Management System:
- Support the administration team by reviewing documents and actioning required follow-ups.
- Ensure clinical correspondence is appropriately managed and communicated.
Collaboration with Multidisciplinary Team:
- Work alongside GPs, nurses, pharmacists, and administrators to enhance patient care.
- Provide advice and guidance to colleagues, supporting best practice within the practice.
- Support GPs in looking after Care Homes:
- New residents and those recently discharged from hospital
- Residents with illness that may need changes to medicines
- Optimising medicines at the end of life
- Other at-risk groups, i.e. on high-risk medicines or at risk of a fall
- Supporting care homes with local policies and procedures, training, vaccinations and any challenges they may encounter.

- Excellent communication and interpersonal skills.
