Job responsibilities
PRINCIPLE
FUNCTIONS:
The post holder will:
- Participate
in the delivery of high standard of nursing/clinical care.
- Work
and liaise with Primary Health Care Team members.
- Be
proficient in the management of chronic disease and implement treatment
plans for patients to manage long term conditions.
- Identify
and manage patients at risk of developing long term conditions
- Advise
women on contraception and sexual health.
- Manage
cervical cytology consultations, obtaining cytology samples in accordance
with the cervical screening programme and competencies.
- Administer
vaccinations and Immunisations for Adults and children following national
programmes.
- Advise,
support and administer vaccinations for patients travelling abroad.
MAIN DUTIES to
include
The provision of nursing treatment and assessment of patients
in participation with the GPs or independently according to agreed and
documented protocols.
Carry out the following when required:
- Asthma
& COPD Reviews
- Spirometry
- Diabetic
Reviews
- Dementia
Reviews
- Venepuncture
- Hypertension
management
- Childhood
immunisation and vaccinations
- Travel
and routine vaccinations
- Administer
prescribed therapies i.e. Vit B12, Depo Provera, neuroleptic medication
etc recognising side effects and
contraindications
- Electrocardiography
- CHD
reviews
- Minor
surgery support including Vasectomy
- Request
pathology tests eg urine culture, swabs etc
- Responsible
for infection control and clinically based protocols including
implementation and training across the practice.
- First
aid and management of an Emergency
- Order
and maintain treatment room stock and vaccinations, ensuring good rotation
and adequate stock levels
- Undertake
home visits if required.
- Attend
and participate in practice/Nurse meetings, as appropriate.
- Participate
in quality assurance processes such as clinical audit, evaluation and
serious incident recording, ensuring any lessons learned are translated
into practice.
- Analyse
data and develop initiatives to improve the uptake of national screening
and immunisation programmes including coordinating new vaccination
campaigns.
- Where
relevant for the nursing team, ensure correspondence and safety alerts are
actioned.
- Maintain
and enforce standards of record keeping and associated administration in
accordance with organisational policy and local joint arrangements and
professional guidelines.
- Recognise
opportunities to reduce inequalities and unwarranted variation in health
care for the practice population, as outlined in the nursing framework

Advise on the promotion of health and the prevention of
illness, through the provision of health education and lifestyle counselling
recording such information as height, weight, smoking status, and alcohol
status onto the EMIS records.

ADMINISTRATION
The post holder must revalidate to maintain their
registration with the NMC and maintain their Continued Professional Development
portfolio.
Maintain clinical and professional knowledge by identifying
training needs at appraisal and attending training and updates in accordance
with individual and Practice requirements.
Liaise with members of the Practices Multi-Disciplinary
Team.
Liaise with the Practice Manager.
It is the responsibility of each individual employee to be
